剖宫产术后患者移植不同发育速度的D3胚胎形成的优质囊胚的妊娠结局比较

摘　　要：目的探讨剖宫产术后患者第3d(D3)胚胎的发育速度对其形成的优质囊胚移植后的妊娠结局影响。方法收集本院生殖医学中心行冻融优质囊胚移植的剖宫产术后患者的临床资料(共107个周期),根据D3胚胎的卵裂球数目分为正常发育组(63个周期)和快速发育组(44个周期),比较两组的妊娠结局。结果正常发育组生化妊娠率及临床妊娠率高于快速发育组(P<0.05);两组的异位妊娠率、流产率、早产率、活产率比较差异无统计学意义(P>0.05)。结论来源于正常发育速度D3胚胎的优质囊胚较来源于快速发育D3胚胎的优质囊胚具有更高的生化妊娠率和临床妊娠率,剖宫产术后患者可优先选择源于正常发育速度D3胚胎的优质囊胚进行移植。Objective To explore the impact of the developmental rate of the third day(D3)embryo in patients undergoing cesarean section on the pregnancy outcomes after high-quality blastocyst transplantation.Methods Clinical data of 107 cycles of cesarean section patients who underwent freeze-thaw high-quality blastocyst transplantation at the Reproductive Medicine Center of our hospital were collected.According to the number of blastomeres in D3 embryos,they were divided into a normal development group(63 cycles)and a rapid development group(44 cycles),and the pregnancy outcomes of the two groups were compared.Results The biochemical pregnancy rate and clinical pregnancy rate in the normal development group were higher than those in the rapid development group(P<0.05);There was no statistically significant difference in the rates of ectopic pregnancy,miscarriage,preterm birth,and live birth between the two groups(P>0.05).Conclusion High quality blastocysts derived from D3 embryos with normal developmental rates have higher biochemical and clinical pregnancy rates than those derived from rapidly developing D3 embryos.Patients undergoing cesarean section can prioritize the transfer of high-quality blastocysts derived from D3 embryos with normal developmental rates.
