自制垫枕在超声引导下肘上PICC置管过程中的应用

摘　　要：目的 探究自制垫枕在超声引导下行肘上经外周静脉穿刺中心静脉(PICC)置管术的临床应用效果。方法 选取2022年1月至2022年12月于赣南医学院第一附属医院肿瘤内科行超声引导下肘上PICC置管的患者120例作为研究对象,采用随机数字表法分为对照组(60例)和试验组(60例)。其中对照组采用临床常规方法进行PICC置管,试验组使用自制垫枕放置于手臂下进行PICC置管。分析比较两组患者血管成像效果、超声探头纵轴与操作台的平面角度、一次性穿刺成功率、置管成功率以及置管时间。结果 试验组患者置管前血管成像效果优于对照组(P<0.05);试验组患者血管成像效果良好时操作台与探头纵轴垂直率高于对照组(P<0.05);对照组患者置管成功率为98.33%、试验组为100.00%,两组对比无统计学意义(P>0.05);试验组患者一次穿刺成功率(98.33%)高于对照组(88.33%)(P<0.05),且多次进针及多次送导丝情况低于对照组(P<0.05);试验组患者的置管时间在30~45 min范围内的例数多于对照组(P<0.05),> 45 min的例数少于对照组(P<0.05)。结论 使用自制垫枕在超声引导下进行肘上PICC置管,能够有效提高血管成像的效果。此外,操作台与超声探头纵轴呈垂直角度更加利于操作,降低了多次进针及送导丝次数,进一步缩短了置管时间。Objective To investigate the clinical application effect of self-made cushion in ultrasound-guided central vein catheterization(PICC)above the elbow.Methods A total of 120 patients who underwent ultrasus-guided suprabucicular PICC catheterization in the Department of Oncology,the First Affiliated Hospital of Gannan Medical College from January 2022 to December 2022 were selected as the study objects.All patients were divided into control group(60 cases)and experimental group(60 cases)by random number table method.Among them,the control group was performed PICC catheterization by clinical routine method,and the experimental group was performed PICC catheterization by self-made cushion placed under the arm.The angiography effect,the plane Angle between the vertical axis of the ultrasonic probe and the operating table,the success rate of one-time puncture,the success rate of catheterization and the catheterization time of the two groups were analyzed and compared.Results The angiography effect of experimental group before catheterization was better than that of control group(P<0.05).The vertical rate of operation table and probe axis in experimental group was higher than that in control group when the angiography effect was good(P<0.05).The success rate of catheterization was 98.33%in the control group and 100.00%in the experimental group,and there was no difference between the two groups(P>0.05).The success rate of one puncture in the experimental group(98.33%)was higher than that in the control group(88.33%)(P<0.05),and the rate of multiple needle insertion and multiple wire delivery was lower than that in the control group(P<0.05).The number of patients with catheterization time within 30-45 min in experimental group was more than that in control group(P<0.05),and the number of patients with catheterization time>45 min was less than that in control group(P<0.05).Conclusions Using self-made cushion to place PICC tube on elbow under ultrasonic guidance can effectively improve the effect of vascular imaging.In addit
