早期肠内营养护理对重症颅脑外伤术后患者营养状况的影响

摘　　要：目的 探究早期肠内营养护理对重症颅脑外伤术后患者营养状况的影响。方法 研究对象为本院2020年1月至2022年12月期间收治的重症颅脑外伤患者共102例,采取随机数字表法分为对照组(常规营养护理)与观察组(早期肠内营养护理),对比两组营养指标,免疫功能水平,血清学相关指标,整体住院时间、ICU住院时间以及患者满意度。结果 护理后观察组血清总蛋白水平、血清白蛋白水平、血清前白蛋白、肱三头肌皮褶厚度、上臂中部肌围等营养指标及CD3^(+)、CD4^(+)、CD8^(+)等免疫功能指标,以及患者及其家属对护理的满意度均高于对照组(均P<0.05);护理后观察组降钙素原、内毒素、C反应蛋白水平等血清学指标均低于对照组(均P<0.05);观察组整体住院时间、ICU住院时间均短于对照组(均P<0.05)。结论 早期肠内营养护理能够提供全面、均衡的营养支持,有助于改善患者的免疫功能水平以及血清学相关指标,减少感染等并发症的发生,缩短患者治疗时间,提高护理满意度。Objective To investigate the impact of early enteral nutrition nursing on the nutritional status of patients with severe craniocerebral trauma after surgery.Methods The study object was a total of 102 cases of patients with severe craniocerebral trauma admitted to our hospital during the period of January 2020-December 2022,which were divided into the control group(conventional nutritional care)and the observation group(early enteral nutritional care)by adopting the method of randomized numerical table,comparing the nutritional indexes,the level of immune function,the serology-related indexes,the overall length of hospital stay,the length of stay in the ICU and the patient satisfaction.Results Nutritional indicators such as serum total protein level,serum albumin level,serum prealbumin,triceps skinfold thickness,and mid-upper arm muscle circumference,as well as immune function indicators such as CD3^(+),CD4^(+),and CD8^(+),as well as patients'and their family members'satisfaction with nursing care were higher in the observation group than in the control group after nursing care(all P<0.05);Serological indicators such as calcitoninogen,endotoxin and C-reactive protein levels in the observation group were lower than those in the control group after care(all P<0.05);the overall hospitalization time and ICU stay in the observation group were shorter than those in the control group(all P<0.05).Conclusions Early enteral nutrition care can provide comprehensive and balanced nutritional support,which can help improve the level of patients'immune function as well as serology-related indexes,reduce the occurrence of infections and other complications,shorten the treatment time of patients,and improve the satisfaction of care.
