心理弹性干预结合前瞻性护理对行游离皮瓣移植修复四肢皮肤软组织缺损患者负性情绪的影响

摘　　要：目的 探究心理弹性干预结合前瞻性护理在行游离皮瓣移植修复四肢皮肤软组织缺损患者中的应用价值。方法 选取晋江市医院2022年1月至2023年1月收治的100例四肢皮肤软组织缺损患者,采用密闭信封法分为对照组(n=50)、研究组(n=50),两组均实施常规护理,对照组结合前瞻性护理,研究组实施心理弹性干预联合前瞻性护理。比较两组手术指标及并发症情况,护理前及护理3个月后,对比两组负性情绪及生活质量评分。结果 两组手术指标对比无统计学意义(P>0.05);研究组并发症发生率为2.00%低于对照组的14.00%(χ^(2)=4.891;P=0.027);研究组汉密尔顿焦虑量表(HAMA)及汉密尔顿抑郁量表(HAMD)评分低于对照组,生活质量简表(WHOQOL-100)评分高于对照组(P<0.05)。结论 在行游离皮瓣移植修复四肢皮肤软组织缺损患者中实施心理弹性干预结合前瞻性护理,可有效缓解患者负性情绪,降低并发症风险,从而提高其生活质量。Objective To explore the application value of psychological resilience intervention combined with prospective nursing in patients with free flap transplantation.Methods A total of 100 patients with skin and soft tissue defects of limbs admitted to Jinjiang Hospital from January 2022 to January 2023 were selected and divided into control group(n=50)and study group(n=50)by sealed envelope method.Both groups implemented routine nursing,the control group combined with prospective nursing,and the research group implemented psychological resilience intervention combined with prospective nursing.The surgical indicators and complications of the two groups were compared,and the scores of negative emotions and quality of life were compared between the two groups before and after 3 months of nursing intervention.Results There was no significant difference between the two groups(P>0.05);The incidence of complications in the study group was 2.00%,lower than 14.00%in the control group(χ^(2)=4.891;P=0.027);the Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD)scores in the study group were lower than those in the control group,and the Quality of Life Brief(WHOQOL-100)scores were higher than those in the control group(P<0.05).Conclusions The implementation of psychological resilience intervention combined with prospective care in patients undergoing free flap transplantation for repair of soft tissue defects in the skin of the extremities can effectively alleviate patients'negative emotions,reduce the risk of complications,and thus improve their quality of life.
