摘 要:目的:探究持续正压通气对阻塞性睡眠呼吸暂停综合征(OSAS)患者睡眠呼吸监测指标和炎症介质水平的影响。方法:选取2021年1月至2022年12月福建省尤溪县总医院呼吸与危重症医学科收治的OSAS患者100例作为研究对象,采用随机数字表法随机分为对照组和观察组,每组50例。对照组给予常规治疗,观察组给予持续正压通气治疗,比较2组治疗前和治疗后相关血清白细胞介素-23(IL-23)及C反应蛋白(CRP)水平、睡眠监测指标及失眠情况。结果:治疗后,2组血清IL-23、CRP水平、呼吸暂停低通气指数(AHI)、最长呼吸暂停时间均较治疗前降低(均P<0.05),且观察组各观察指标均低于对照组(均P<0.05)。治疗后,2组呼吸暂停指数、AHI、最长呼吸暂停时间均较治疗前下降(均P<0.05),且观察组各指标均低于对照组(均P<0.05)。治疗后2组失眠严重程度指数(ISI)评分较治疗前降低(P<0.05),且观察组ISI评分低于对照组(P<0.05)。结论:持续正压通气能够有效降低OSAS患者血清炎症介质水平,改善睡眠呼吸暂停情况,缓解失眠症状。Objective:To explore the effects of continuous positive airway pressure(CPAP)ventilation on sleep respiratory monitoring indices and levels of inflammatory markers in patients with Obstructive Sleep Apnea Syndrome(OSAS).Methods:A hundred OSAS patients admitted to the Department of Respiratory and Critical Care Medicine at the General Hospital of Youxi County,Fujian Province,from January 2021 to December 2022 were selected for this study.Using a random number table method,patients were divided into a control group and an observation group,with 50 cases in each.The control group received conventional treatment,while the observation group underwent CPAP therapy.Comparisons were made between the two groups before and after treatment in terms of serum levels of interleukin-23(IL-23)and C-reactive protein(CRP),sleep monitoring indices,and insomnia conditions.Results:After treatment,both groups showed reductions in serum IL-23,CRP levels,apnea-hypopnea index(AHI),and the longest apnea duration compared to before treatment(P s<0.05),with the observation group showing lower levels in all observed indices than the control group(P s<0.05).The severity of insomnia,as measured by the Insomnia Severity Index(ISI),also decreased in both groups after treatment(P s<0.05),with the observation group reporting lower ISI scores than the control group(P<0.05).Conclusion:Continuous positive pressure ventilation can effectively reduce serum levels of inflammatory markers,improve sleep apnea conditions,and alleviate symptoms of insomnia in patients with Obstructive Sleep Apnea Syndrome.
