摘　　要：目的探究血清晚期氧化蛋白产物(AOPP)、单核细胞趋化蛋白-1(MCP-1)水平与妊娠期糖尿病(GDM)孕妇子痫前期(PE)发生的相关性。方法选取2021年10月至2023年9月广西医科大学附属民族医院产科收治的70例GDM并发PE孕妇作为合并组,另选取同期来广西医科大学附属民族医院孕检的单纯GDM孕妇70例作为非合并组,统计分析两组研究对象的临床资料及实验室指标,采用多因素logistic分析血清AOPP、MCP-1与GDM并发PE的相关性。选用ROC曲线分析血清AOPP、MCP-1联合检测GDM并发PE的诊断价值。结果单因素分析结果显示,两组患者在年龄、孕前体重指数(BMI)、血清AOPP以及MCP-1表达水平上的比较,差异均具有统计学意义(P<0.05);多因素logistic回归显示,年龄、孕前BMI、血清AOPP以及MCP-1均为GDM并发PE的独立危险因素(P<0.05)。ROC曲线显示,血清AOPP、MCP-1及二者联合检测曲线下面积为0.867、0.916、0.942。结论AOPP、MCP-1与GDM并发PE发病风险密切相关,对病情诊断具有较高应用价值。Objective To explore the correlation between serum levels of late oxidized protein product(AOPP),monocyte chemoattractant protein-1(MCP-1)and preeclampsia(PE)in pregnant women with gestational diabetes mellitus(GDM).Methods A total of 70 pregnant women with GDM complicated with PE admitted to the obstetrics Department of the Affiliated Hospital for Nationalities of Guangxi Medical University from October 2021 to September 2023 were selected as the combination group,and another 70 pregnant women with simple GDM who came to the Affiliated Hospital for Nationalities,Guangxi Medical University during the same period were selected as the non-combination group.The clinical data and laboratory indicators of the two groups of subjects were statistically analyzed.Multivariate logistic analysis was used to analyze the correlation between AOPP,MCP-1 and GDM complicated with PE.ROC was used to analyze the diagnostic value of serum AOPP and MCP-1 combined detection of GDM complicated with PE.Results Univariate analysis showed that there were statistically significant differences in age,pre-pregnancy body mass index(BMI),serum AOPP and MCP-1 expression levels between the two groups(P<0.05).Multivariate logistic regression showed that age,pre-pregnancy BMI,serum AOPP and MCP-1 were independent risk factors for GDM complicated with PE(P<0.05).ROC curve showed that the areas under serum AOPP,MCP-1 and their combined detection curves were 0.867,0.916 and 0.942.Conclusion AOPP and MCP-1 are closely related to the risk of GDM with PE,and have high application value in the diagnosis of the disease.
