摘　　要：目的探讨钛镍记忆合金线分次牵张联合负压封闭引流(VSD)修复慢性下肢皮肤缺损创面的治疗效果。方法回顾性分析2019年6月至2022年12月广州医科大学附属第五医院应用钛镍记忆合金线分次牵拉联合VSD修复30例慢性下肢小面积全层皮肤缺损创面患者的临床资料。创面缺损面积1.5cm×2.0cm~3.0cm×13.0 cm,观察患者术后伤口愈合情况,记录并发症及随访结果。结果住院时间8~35 d(平均16.8 d);所有患者伤口愈合均良好,愈合时间19~31 d(平均24.5 d),外观良好;术后和随访期间未出现创面裂开或感染。结论钛镍记忆合金线分次牵张联合VSD治疗一期无法缝合的下肢小面积皮肤全层缺损慢性创面简便有效,具有愈合时间短、外观良好等优点。Objective To investigate the therapeutic effect of TiNi memory alloy suture fractional stretching combined with vacuum sealing drainage(VSD)in repairing chronic lower limb skin defect wounds.Methods Clinical data of 30 patients with chronic full-layer skin defect wounds of lower limbs treated in the Fifth Affiliated Hospital of Guangzhou Medical University from June 2019 to December 2022 were retrospectively analyzed.TiNi memory alloy suture fractional traction combined with VSD was used to repair the defect wounds(the defect area,1.5 cm×2.0 cm to 3.0 cm×13.0 cm).Postoperative wound healing was observed,and the complications and follow-up results were recorded.Results Average of hospital stay was 16.8 d(8 to 35 d).Wound healing was good in all patients,with healing time of 19 to 31 d(average,24.5 d).Good appearance was also found in 30 cases.No wound dehiscence or infection was found at postoperation and during the follow-up.Conclusion Being a simple and convenient surgical procedure,TINi memory alloy suture fractional stretching combined with VSD is effective in the treatment of chronic wounds of small full-layer skin defects of lower limbs which cannot be sutured in the first stage,and has the advantages of short healing time and good appearance.
