血清AOPP、LCN-2水平在妊娠期糖尿病孕妇不良妊娠结局中的预测分析

摘　　要：目的:研究血清晚期氧化蛋白产物(Gestational diabetes mellitus,AOPP)、人脂质运载蛋白2(Lipocalin 2,LCN-2)水平在妊娠期糖尿病(Gestational diabetes mellitus,GDM)孕妇不良妊娠结局中的预测价值。方法:回顾性收集2021年10月-2022年6月在我院分娩的89例GDM孕妇的临床资料。根据妊娠结局将孕妇分为妊娠结局良好组(n=67)和妊娠结局不良组(n=22)。分析不良妊娠结局情况;对比两组的血清AOPP、LC-2水平;分析血清AOPP、LCN-2水平与GDM孕妇不良妊娠结局的相关性及其对不良妊娠结局的预测价值。结果:妊娠结局不良组血清AOPP、LCN-2较妊娠结局良好组显著提高(P<0.05)。经双变量Pearson分析,血清AOPP、LCN-2水平与GDM孕妇不良妊娠结局呈正相关(r=0.568、0.599,P<0.001)。绘制ROC曲线显示,得到AUC分别为:0.852、0.890、0.915,其中联合检验的预测价值最高。结论:GDM孕妇血清AOPP、LCN-2高表达与不良妊娠结局具有密切关系,且具有一定预测价值。Objective:To investigate the predictive value of serum levels of late oxidized protein product(AOPP)and human lipocalin 2(LCN-2)in adverse pregnancy outcomes in pregnant women with gestational diabetes mellitus(GDM).Methods:The clinical data of 89 pregnant women with GDM who delivered in our hospital from October 2021 to June 2022 were retrospectively collected.According to pregnancy outcome,pregnant women were divided into good pregnancy outcome group(n=67)and bad pregnancy outcome group(n=22).Analysis of adverse pregnancy outcomes;The serum AOPP and LC-2 levels were compared between the two groups.To analyze the correlation between serum AOPP and LCN-2 levels and adverse pregnancy outcomes in GDM pregnant women and their predictive value for adverse pregnancy outcomes.Results:The serum AOPP and LCN-2 in the adverse pregnancy outcome group were significantly higher than those in the good pregnancy outcome group(P<0.05).By bivariate Pearson analysis,serum AOPP and LCN-2 levels were positively correlated with adverse pregnancy outcomes in GDM pregnant women(r=0.568,0.599,P<0.001).ROC curve was drawn to show that the AUC was 0.852,0.890 and 0.915,respectively,in which the predictive value of the combined test was the highest.Conclusion:The high expression of serum AOPP and LCN-2 in pregnant women with GDM is closely related to adverse pregnancy outcomes,and has certain predictive value.
