关节镜下髌骨外侧支持带松解联合微骨折术治疗髌股关节炎的临床疗效研究

摘　　要：目的:研究关节镜下髌骨外侧支持带松解联合微骨折术治疗髌股关节炎的临床疗效。方法:选取2022年1月—2022年12月祁阳市人民医院收治的72例髌股关节炎患者作为研究对象,按照奇偶数原则分为参照组和试验组,各36例。参照组采用关节清理灌洗术治疗,试验组采用关节镜下髌骨外侧支持带松解联合微骨折术治疗。对比两组治疗前后疼痛程度、膝关节功能、治疗总有效率、并发症发生率。结果:术后3、6个月,两组疼痛评分均低于术前,且试验组低于参照组,差异有统计学意义(P<0.05)。术后3、6个月,两组膝关节功能评分均高于术前,且试验组高于参照组,差异有统计学意义(P<0.05)。试验组患者治疗后总有效率高于参照组,差异有统计学意义(P<0.05)。试验组患者并发症发生率低于参照组,差异有统计学意义(P<0.05)。结论:在髌股关节炎治疗中应用关节镜下髌骨外侧支持带松解联合微骨折术治疗的临床疗效显著,能够降低患者的关节疼痛程度和并发症发生风险,促进膝关节功能水平恢复。Objective:To study the clinical effect of arthroscopic release of lateral patellar retinaculum combined with microfracture in the treatment of patellofemoral arthritis.Methods:A total of seventy-two patients with patellofemoral arthritis admitted to Qiyang People's Hospital from January 2022 to December 2022 were selected as the study objects,and were divided into the reference group and the experimental group according to the odd and even principle,with thirty-six cases in each group.The reference group was treated with joint cleaning and lavage,and the experimental group was treated with arthroscopic release of lateral patellar retinaculum combined with microfracture.The pain degree,knee function,total effective rate and complication rate of the two groups were compared before and after treatment.Results:At 3 and 6 months after surgery,the pain scores in both groups were lower than before surgery,and the experimental group was lower than the reference group,with statistical significance(P<0.05);at 3 and 6 months after surgery,the knee function scores of both groups were higher than those before surgery,and the experimental group was higher than the reference group,with statistical significance(P<0.05);the total effective rate of experimental group was higher than that of reference group,and the difference was statistically significant(P<0.05);the complication rate of experimental group was lower than that of reference group,with statistical significance(P<0.05).Conclusion:In the treatment of patellofemoral arthritis,arthroscopic lateral patellar retinaculum combined with microfracture has a significant clinical effect,which can reduce the degree of joint pain and the risk of complications,and promote the recovery of knee function.
