摘　　要：从“态靶辨治”角度介绍蔡淦教授治疗脾虚瘀热型胃癌前病变的经验。认为其基本病机为脾失健运,湿阻热蕴,气滞血瘀胶着于胃腑;临证强调“态靶辨治”,把握“基本态”、紧扣“疾病靶”、把控“症状靶”、参考“指标靶”;治疗时灵活运用经验方莪连颗粒健脾益气、化瘀清热以调态,并通过抗幽门螺杆菌、调节炎症因子及肿瘤指标表达水平、提高机体免疫力、影响相关信号通路等途径“精确打靶”,疗效突出。并附验案1则。Professor Cai Gan's experience in treating precancerous lesions of gastric cancer(PLGC)with spleen deficiency and stasis-heat syndrome is introduced from the perspective of state-targeted treatment.It is held that the fundamental pathogenesis involves the failure of spleen in transporting and transforming,leading to dampness obstruction,heat accumulation,and qi stagnation and blood stasis intermingling in the stomach.In clinical practice,the emphasis is on state-targeted treatment,focusing on grasping the basic state,closely monitoring the disease target,controlling the symptom target,and referring to the index target.The treatment strategy uses the experiential formula E Lian Granule,which invigorates the spleen and supplements qi,dispels stasis and clears heat,thereby modulating the state.The approach precisely adopts the targeted therapy through anti-Helicobacter pylori measures,regulating inflammatory factors and tumor marker expression levels,enhancing the body's immune response,and influencing relevant signaling pathways.The therapeutic effect is notable.One proven case is attached for demonstration.
