摘　　要：目的探讨多学科疼痛管理对老年髋关节置换术患者康复及衰弱的影响。方法采用非同期对照研究,将南华大学附属第二医院2020年1~6月接受髋关节置换术的48例患者作为对照组,采用急性疼痛服务模式;2020年7~12月接受治疗的48例患者作为干预组,采用多学科疼痛管理模式。比较两组术后疼痛情况、术后镇痛不良反应发生例数、术后康复指标(首次下床活动时间、住院时间、髋关节功能评分)与衰弱状况。结果干预组术后24 h疼痛评分、中重度疼痛发生例数、术后镇痛不良反应例数均低于对照组,首次下床活动时间及住院时间较对照组缩短,髋关节功能评分提高,衰弱及衰弱前期例数减少,两组差异均有统计学意义(均P<0.05)。结论多学科疼痛管理能缓解老年髋关节置换术患者术后疼痛,促进快速康复,改善衰弱状况。Objective To investigate the effect of multidisciplinary pain management on rehabilitation and frailty in elderly patients with hip replacement.Methods In this study,48 patients who underwent hip replacement in the hospital from January to June 2020 were selected as the control group,and acute pain service mode was adopted.A total of 48 patients who received treatment from July to December 2020 served as the intervention group and adopted a multidisciplinary pain management model.The postoperative pain,the number of postoperative analgesia adverse reactions,postoperative rehabilitation indexes(first time of getting out of bed,length of stay,hip function score)and frailty status were compared between the two groups.Results The 24 hours postoperative pain score,the number of cases of moderate and severe pain,and the number of postoperative analgesia adverse reactions in the intervention group were lower than those in the control group,the first time of getting out of bed and the length of hospital stay were shorter than those in the control group,the hip function score was increased,and the number of asthenia and pre-asthenia cases was decreased,with statistical significance between the two groups(P<0.05).Conclusions The multidisciplinary team for pain management mode relieved postoperative pain,promoted enhanced recovery and ameliorated frailty conditions in elderly patients treated with hip arthroplasty.
