摘 要:目的探究肺炎支原体肺炎(MPP)患儿血清金属蛋白酶组织抑制因子3(TIMP3)和超氧化物歧化酶2(SOD2)水平与肠道菌群及免疫功能的关系。方法选取MPP患儿120例作为研究对象,分为轻症组65例和重症组55例,同时选取120例在本院查体的健康儿童作为对照组,比较三组血清TIMP3、SOD2水平。Pearson法分析血清TIMP3、SOD2水平与肠道菌群及免疫功能的相关性。结果与对照组相比,轻症组和重症组血清TIMP3水平显著降低,SOD2及D-乳酸水平升高,粪便中双歧杆菌、大肠杆菌、乳酸杆菌、链球菌水平依次降低,免疫功能水平降低(P均<0.05)。相关性分析显示,MPP患儿血清TIMP3分别与双歧杆菌、大肠杆菌、乳酸杆菌、IgG、CD4^(+)/CD8^(+)呈正相关性,与D-乳酸、hs-CRP、TNF-α、WBC呈负相关性;SOD2与双歧杆菌、大肠杆菌、乳酸杆菌、IgG、IgM、补体C3、补体C4、CD4^(+)/CD8^(+)呈负相关性,与D-乳酸、hs-CRP、TNF-α、WBC呈正相关性(P<0.05)。结论重症MPP患儿血清TIMP3水平降低,SOD2水平升高,并且二者与MPP患儿肠道菌群及免疫功能密切相关。Objective To investigate the relationship between serum tissue inhibitor of metalloproteinase 3(TIMP3),superoxide dismutase 2(SOD2)levels and intestinal flora and immune function in children with mycoplasma pneumoniae pneumonia(MPP).Methods A total of 120 children with MPP admitted to our hospital from July 2019 to July 2022 were collected as the study subjects,who were divided into the mild illness group of 65 cases and the severe illness group of 55 cases.At the same time,120 healthy children who underwent physical examination in our hospital were collected as the control group,and the serum levels of TIMP3 and SOD2 were compared among the three groups.Pearson method was applied to analyze the correlation between serum levels of TIMP3 and SOD2 and intestinal flora and immune function.Results Compared with the control group,the serum levels of TIMP3 and SOD2 in the mild and severe groups were obviously lower(P<0.05).Compared with the control group,the levels of bifidobacteria,Escherichia coli,lactobacilli,and streptococci in the feces of children in the mild illness group and the severe illness group decreased sequentially(P<0.05).The D-lactic acid of children in the severe group was higher than that in the mild group and the control group(P<0.05).Compared with the control group,the level of immune function in the mild group and the severe group decreased(P<0.05).Correlation analysis showed that serum TIMP3 in children with MPP was positively correlated with bifidobacterium,Escherichia coli,lactobacillus,IgG,CD4^(+)/CD8^(+),respectively,and negatively correlated with D-lactic acid,hs-CRP,TNF-αand WBC(P<0.05).Serum SOD2 was negatively correlated with bifidobacteria,Escherichia coli,lactobacillus,IgG,IgM,C3,C4 and CD4^(+)/CD8^(+),respectively,and positively correlated with D-lactic acid,hs-CRP,TNF-α,and WBC(P<0.05).Conclusion In children with severe MPP,the level of serum TIMP3 decreases and the level of SOD2 increases,and both are closely related to intestinal flora and immune function in children with MPP.
