摘  要:目的 对肠道微生态失衡与小儿支气管肺炎细胞免疫的相关性进行探究。方法 选取2021年3月-2023年3月在本院就诊的105例支气管肺炎患儿作为肺炎组,根据患儿疾病严重程度分为轻症肺炎组(n=74)与重症肺炎组(n=31),选取同期在我院体检健康的105名儿童为对照组,比较各组肠道菌群及细胞免疫指标水平。Pearson法分析肠道菌群与细胞免疫的相关性。结果 肺炎组患儿双歧杆菌[(6.33±1.08)CFU/g vs(7.21±1.25)CFU/g]、乳酸杆菌[(7.85±1.20)CFU/g vs(8.64±1.18)CFU/g]水平较对照组低,大肠杆菌[(7.14±1.13)CFU/g vs(6.25±0.98)CFU/g]、链球菌[(6.12±1.06)CFU/g vs(5.28±0.76)CFU/g]水平较对照组高,差异具有统计学意义(P均<0.05)。重症组患儿双歧杆菌[(5.40±0.64)CFU/g vs(6.72±0.71)CFU/g]、乳酸杆菌[(7.42±0.97)CFU/g vs(8.03±1.04)CFU/g]水平较轻症组低,大肠杆菌[(7.78±0.95)CFU/g vs(6.87±0.78)CFU/g]、链球菌[(7.31±1.06)CFU/g vs(5.62±0.66)CFU/g]水平较轻症组高,差异具有统计学意义(P均<0.05)。肺炎组患儿CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平比对照组低,CD8^(+)、IgA、IgM、IgG水平较对照组高,差异具有统计学意义(P均<0.05)。与轻症组相比,重症组患儿CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平降低,CD8^(+)、IgA、IgM、IgG水平升高,差异具有统计学意义(P均<0.05)。相关性分析显示,双歧杆菌、乳酸杆菌分别与CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)呈正相关,与CD8^(+)、IgA、IgM、IgG呈负相关;大肠杆菌、链球菌分别与CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)呈负相关,与CD8^(+)、IgA、IgM、IgG呈正相关。结论 支气管肺炎患儿的肠道微生态失衡与细胞免疫关系密切,不同疾病严重程度患儿的肠道菌群及细胞免疫功能指标差异显著。Objective To explore the correlation between intestinal microbiota imbalance and cellular immunity in children with bronchopneumonia.Methods A total of 105 children with Mycoplasma pneumonia who visited our hospital from March 2021 to March 2023 were regarded as the pneumonia group.According to the severity of the disease,the children were separated into the mild pneumonia group(74 cases)and the severe pneumonia group(31 cases).Another 105 healthy children who underwent physical examination in our hospital were regarded as the control group.The levels of gut microbiota and cellular immune indicators in each group were compared.Pearson method was applied to analyze the correlation between gut microbiota and cellular immunity.Results The levels of bifidobacterium[(6.33±1.08)CFU/g vs(7.21±1.25)CFU/g]and lactobacillus[(7.85±1.20)CFU/g vs(8.64±1.18)CFU/g]in the pneumonia group were lower than those in the control group,while the levels of Escherichia coli[(7.14±1.13)CFU vs(6.25±0.98)CFU/g]and streptococcus[(6.12±1.06)CFU/g vs(5.28±0.76)CFU/g]were higher than those in the control group(all P<0.05).The levels of bifidobacteria[(5.40±0.64)CFU/g vs(6.72±0.71)CFU/g]and lactobacilli[(7.42±0.97)CFU/g vs(8.03±1.04)CFU/g]in the severe group were lower than those in the mild group,while the levels of Escherichia coli[(7.78±0.95)CFU/g vs(6.87±0.78)CFU/g]and streptococcus[(7.31±1.06)CFU/g vs(5.62±0.66)CFU/g]were higher than those in the mild group(all P<0.05).The levels of CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in the pneumonia group were lower than those in the control group,while the levels of CD8^(+),IgA,IgM,and IgG were higher than those in the control group(P<0.05).Compared with the mild group,the levels of CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)in the severe group decreased,while the levels of CD8^(+),IgA,IgM,and IgG increased(P<0.05).Correlation analysis showed that bifidobacterium and lactobacillus were positively correlated with CD3^(+),CD4^(+),CD4^(+)/CD8^(+),and negatively correlated with CD8^(+),IgA,IgM,and IgG,re
