摘　　要：目的 探究固有淋巴样细胞(ILCs)在初治肺结核(PTB)疾病中的亚群变化,并分析其对体内炎症水平的影响,为更有效的结核杆菌感染防治策略提供科学支持。方法 选取临床诊断为初治肺结核的患者40例(肺结核组)和健康者40例(对照组),收集其临床资料及外周血。流式细胞术(FCM)检测循环ILCs、ILC亚群的占比。采用酶联免疫吸附试验(ELISA)检测血清CXCL13、IFN-γ、IL-23、IL-17、IL-22。结果 与对照组相比,肺结核组中ILC1和ILC1/ILC3值显著下调(P<0.001),而ILC3值显著上调(P<0.001)。在肺结核组和对照组之间,ILC2和总ILC值的比较差异无统计学意义。肺结核组血清中CXCL13、IFN-γ、IL-23和IL-17水平显著高于对照组(P<0.01),而IL-22在两组之间差异无统计学意义。结论 在初治肺结核患者中,外周血ILC3上调,ILC3和ILC1比例失调,并表现CXCL13、IL-23、IL-17高表达。Objective To investigate the subpopulation changes of innate lymphoid cells(ILCs)in initial pulmonary tuberculosis(PTB)disease and to analyze their effect on the level of inflammation in vivo,in order to provide scientific support for a more effective strategy for the prevention and treatment of Mycobacterium tuberculosis infection.Methods 40 patients clinically diagnosed with initial pulmonary tuberculosis(PTB group)and 40 healthy individuals(control group)were selected and their clinical data and peripheral blood were collected.Flow cytometry(FCM)was performed to detect the percentage of circulating ILCs,and ILC subgroups.Serum CXCL13,IFN-γ,IL-23,IL-17 and IL-22 were detected by enzyme linked immunosorbent assay(ELISA).Results ILC1 and ILC1/ILC3 values were significantly down-regulated(P<0.001)and ILC3 values were significantly upregulated(P<0.001)in PTB group compared to control group.There was no significant difference in ILC2 and total ILC values between PTB group and control group.Serum levels of CXCL13,IFN-γ,IL-23 and IL-17 were significantly higher in PTB group than in control group(P<0.01),while IL-22 showed no significant difference between two groups.Conclusion In patients with initial pulmonary tuberculosis,peripheral blood ILC3 was upregulated,ILC3 and ILC1 were disproportionately represented,and exhibited high expression of CXCL13,IL-23,and IL-17.
