妊娠中期血清氨基酸水平与妊娠期糖尿病的相关性研究

摘  要:目的探讨妊娠中期血清氨基酸水平与妊娠期糖尿病(gestational diabetes mellitus,GDM)之间的关系。方法选取2021年1月~2022年12月来佛山市妇幼保健院产检的48例GDM孕妇(GDM组)和55例非GDM孕妇(非GDM组)分组进行队列研究分析,比较两组的基本资料并对两组22种氨基酸水平进行差异性分析;随后以妊娠期糖尿病为因变量,差异氨基酸为自变量进行二元Logistics回归,找出与GDM相关的主要氨基酸。结果两组的基本资料差异无统计学意义(P>0.05)。GDM组中谷氨酸(Glu)、甘氨酸(Gly)、脯氨酸(Pro)、丙氨酸(Ala)、天冬酰胺(Asn)、亮氨酸(Leu)、蛋氨酸(Met)、苯丙氨酸(Phe)、酪氨酸(Tyr)、缬氨酸(Val)水平均值均高于非GDM组,差异有统计学意义(P<0.05);二元Logistics回归排除混杂因素,Ala水平升高是GDM独立的风险因素[OR=1.041,95%CI=(1.011,1.072),P=0.007]。结论GDM的发展及转归与氨基酸代谢密切相关,氨基酸变化可能对GDM的病情发展及转归有着重要提示作用,代谢组学的分析方法有望为GDM的研究提供了新的思路与参考。Objective To investigate the relationship between serum amino acid levels in the second trimester of pregnancy and gestational diabetes mellitus(GDM).Methods A total of 48 pregnant women with GDM(GDM group)and 55 pregnant women without GDM(non-GDM group)were selected from Foshan Women and Children Hospital from January 2021 to December 2022 for cohort analysis.The basic data comparation of the two groups and the difference analysis of 22 amino acids were performed.Subsequently,GDM was taken as the dependent variable and differential amino acids as the independent variable for Binary Logistics regression to find out the main amino acids related to GDM.Results There was no significant difference in the baseline covariates between the two groups(P>0.05).The levels of glutamic acid,glycine,proline,alanine,asparagine,leucine,methionine,phenylalanine,tyrosine and valine in GDM group were higher than those in non-GDM group,the differences were statistical significant(P<0.05).Confounders were excluded by Binary Logistics regression.Elevated Ala levels emerged as an independent risk factor for GDM[OR=1.041,95%CI=(1.011,1.072),P=0.007].Conclusion The development and outcome of GDM are closely related to amino acid metabolism,and the changes of amino acid may be an important indicator for the progression and outcome of GDM.The metabolomics analysis method is expected to provide new vision and reference for the study of GDM.
