摘　　要：目的了解左心室辅助装置(LVAD)植入术后患者及其主要照护者居家照护需求。方法采用目的抽样法,选取2022-02-28至2022-12-27在南京市第一医院心胸血管外科接受LVAD植入术的门诊患者及其主要照顾者作为研究对象。采用描述性现象学研究法进行质性研究:首先,制定访谈提纲;然后,告知患者及其主要照护者LVAD植入术后3个月来南京市第一医院心胸血管外科复诊,并于心胸血管外科病区会议室接受面对面半结构访谈,样本量以资料饱和为标准,本研究最终纳入10例受访者;之后收集资料并采用“Colaizzi现象学分析法”分析资料。主题由所有研究人员讨论得出,若存在不一致意见,则由2名研究人员对初次受访者进行二次访谈,访谈时间为LVAD植入术后4个月,访谈地点及资料收集、分析方法同第一次访谈。结果通过两次访谈,共提炼出3个核心主题13个亚主题,分别为自我管理需求(伤口换药、电源管理、报警识别及处理、用药指导、并发症识别、急救技能)、运动康复需求(居家运动康复时机、居家运动康复方法)及社会支持需求(个人生活需求、复诊需求、经济帮助需求、心理支持需求及回归社会需求)。结论LVAD植入术后患者及其主要照护者在伤口换药、电源管理、报警识别及处理、用药指导、并发症识别、急救技能方面存在需求,其不了解居家运动康复时机、方法,且其社会支持需求广泛。Objective To understand the home care needs of patients and their primary caregivers after left ventricular assist device(LVAD)implantation.Methods The objective sampling method was used to select outpatients and their primary caregivers who underwent LVAD implantation in the Department of Cardiothoracic Vascular Surgery,Nanjing First Hospital from February 28,2022 to December 27,2022 as the research objects.Descriptive phenomenological research method was used to conduct qualitative research:first,the interview outline was developed;then,the patients and their primary caregivers were informed to return to the Cardiothoracic Vascular Surgery Department of Nanjing First Hospital 3 months after LVAD implantation,and received face-to-face semi-structured interviews in the conference room of the Cardiothoracic Vascular Surgery Ward,the sample size was based on data saturation,and 10 respondents were finally included in this study;after that,data were collected and analyzed by"Colaizzi phenomenological method".The topic was discussed by all researchers.If there was disagreement,two researchers conducted a second interview with the first interviewer.The interview time was 4 months after LVAD implantation.The interview location,data collection and analysis methods were the same as those of the first interview.Results Through two interviews,a total of 3 core themes and 13 sub-themes were extracted,including self-management needs(wound dressing,power management,alarm identification and processing,medication guidance,complication identification,first aid skills),sports rehabilitation needs(timing and methods of home sports rehabilitation)and social support needs(personal life needs,referral needs,economic help needs,psychological support needs and return to society needs).Conclusion Patients after LVAD implantation and their primary caregivers have needs in wound dressing,power management,alarm identification and processing,medication guidance,complication identification,and first aid skills.They do not understand the timi
