摘 要:目的探究儿童合并症/并发症优化分组,为完善DRG分组提供实证依据。方法以2017年-2019年北京市属医院住院患儿病案首页数据为研究对象,利用2017年版CN-DRG分组器对17个儿童DRG组进行合并症/并发症亚组细分,运用变异系数(CV)和总体方差减少系数(RIV)进行效度评价。结果ES10、EX10、IC30、IE10、IF10、IF20、IH30对应的细分DRG组CV值<1.00、RIV值>99.00%、秩和检验P<0.05,优化分组成立。结论儿童DRG合并症/并发症优化分组有利于提高儿童病例分组精准度,可为精确付费提供保障。Objective To explore the optimal grouping of pediatric comorbidities/complications,so as to provide empirical basis for improving diagnosis related groups(DRG).Methods Based on the data from the first page of the medical records of hospitalized children in Beijing municipal hospitals from 2017 to 2019,17 DRG groups were subdivided into subgroups of comorbidities/complications by CN-DRG grouping in 2017,and coefficient of variation(CV)and coefficient of total variance reduction(RIV)were used for validity evaluation.Results The CV values of the subdivided DRG groups corresponding to ES10,EX10,IC30,IE10,IF10,IF20 and IH30 were all below 1,RIV values were all higher than 99.00%,and P<0.05 was observed in all cases.The optimization groups were established.Conclusion The optimization of pediatric DRG comorbidities/complications grouping is beneficial to improve the accuracy of pediatric case grouping and provide guarantee for accurate payment.
