摘  要:目的探讨糖尿病生化检验的诊断价值。方法选取2021年6月—2023年6月苏州市吴中区第二人民医院收治的50例糖尿病患者设为观察组,同期选取50例健康体检者设为对照组。两组对象均自愿配合调查与研究,进行生化检验,检测方法为化学发光免疫测定,检测指标包括空腹血糖、餐后2 h血糖、胰岛素水平、C肽。比较两组对象的血糖指标(空腹血糖与餐后2 h血糖)、胰岛素与C肽(空腹、餐后1 h、餐后2 h)水平;对比单一诊断与联合诊断的诊断效能。结果观察组空腹血糖为(8.42±1.02)mmol/L,餐后2 h血糖为(11.16±1.62)mmol/L,低于对照组的(4.62±0.72)、(6.52±0.76)mmol/L,差异有统计学意义(t=21.521、18.335,P均<0.001)。观察组空腹、餐后2 h时胰岛素与C肽水平高于对照组,而餐后1 h胰岛素与C肽水平低于对照组,差异有统计学意义(P均<0.05)。临床最终诊断结果显示阳性50例、阴性50例,指标联合诊断在诊断灵敏度、准确度与特异度上均高于单一指标诊断,差异有统计学意义(P均<0.05)。结论可根据空腹餐后2 h血糖及空腹、餐后1 h、餐后2 h胰岛素水平与C肽水平进行病情评估,同时这些指标联合诊断效能要明显优于单一指标诊断。Objective To investigate the diagnostic value of biochemical test in diabetes.Methods 50 patients with diabetes admitted to Suzhou Wuzhong District Second People's Hospital from June 2021 to June 2023 were selected as the observation group.In the same period,50 healthy objects were treated as control group.Both groups volunteered to cooperate in the investigation and research,and performed biochemical tests by chemiluminescence immunoassay.The measures included fasting blood glucose,2 h postprandial blood glucose,insulin level and C-peptide.Blood glucose indicators(including fasting blood glucose and 2 h postprandial blood glucose)were compared between the two groups.Insulin and C-peptide were compared between the two groups(including fasting,1 h,2 h postprandial).The diagnostic efficiency of single diagnosis and combined diagnosis of different indexes were compared.Results The fasting blood glucose(8.42±1.02)mmol/L and 2 h postprandial blood glucose(11.16±1.62)mmol/L in the observation group were lower than those in the control group(4.62±0.72)mmol/L and(6.52±0.76)mmol/L,the differences were statistically significant(t=21.521,18.335,both P<0.001).The levelsof insulin and C-peptide at fasting and 2 h after meal in the observation group were higher than those in the control group,while the levels of insulin and C-peptide at 1 h after meal were lower than those in the control group,the differences were statistically significant(all P<0.05).The final clinical diagnosis results showed that 50 cases were positive and 50 cases were negative.The sensitivity,accuracy and specificity of combined diagnosis were higher than those of single index diagnosis,and the differences were statistically significant(all P<0.05).Conclusion The diabetes can be evaluated according to the fasting and 2 h postprandial blood glucose and the fasting,1 h,2 h postprandial insulin level and C-peptide level.Meanwhile,the combined diagnostic efficacy of these indicators is significantly better than that of a single indicator.
