探讨TRACK-C评分在0~3岁哮喘婴幼儿不同患病时期的应用价值研究

摘  要:目的探讨改良中文版儿童呼吸和哮喘控制测试(Test for Respiratory and Asthma Control in Kids Chinese Version,TRACK-C)在0~3岁哮喘婴幼儿不同患病时期的应用价值。方法选取2017年8月—2023年1月空军军医大学唐都医院小儿哮喘门诊中已建立标准化电子病历的203名0~3岁哮喘患儿作为哮喘组,同期100名健康儿童作为对照组。比较各组的TRACK-C评分、潮气肺功能、呼出气一氧化氮(Fractional Exhaled Nitric Oxide,FeNO)水平并进行相关性分析。结果哮喘组的FeNO、呼吸频率(Respiratory Rate,RR)和血液IgE水平高于对照组,达峰值时间比(Time to Peak Expirtory Flow/Time of Expiratory,TPTEF/TE)、达峰容积比(Volume to Peak Expiratory Flow/Volume of Expiratory,VPEF/VE)、公斤潮气量(Tidal Volume/kg,VT/KG)、TRACK-C评分水平低于对照组,差异有统计学意义(P均<0.05)。哮喘组急性期FeNO水平、RR水平高于缓解期,且TPTEF/TE、VPEF/VE、VT/KG、TRACK-C评分水平低于缓解期,差异有统计学意义(P均<0.05)。采用Pearson分析哮喘不同患病时期与各项观察指标的相关性,结果显示哮喘不同患病时期与TPTEF/TE、VPEF/VE、TRACK-C评分呈负相关(r=-0.468、-0.452、-0.289,P均<0.05)。结论TRACK-C评分可充分应用于0~3岁哮喘幼儿的病情评估,与潮气肺功能结合可以更好地对低龄患儿进行哮喘管理。Objective To explore the application value of the improved Test for Respiratory and Asthma Control in Kids Chinese Version(TRACK-C)in different periods of asthma in 0 to 3 years old infants.Methods A total of 203 children with asthma aged 0 to 3 who had established standardized electronic medical records in the pediatric asthma clinic of Tangdu Hospital of Air Force Military Medical University from August 2017 to January 2023 were selected as the asthma group and 100 healthy children as the control group in the same period.TRACK-C score,tidal pulmonnary function and fractional exhaled nitric oxide(FeNO)level among all groups were compared and correlation analysis was performed.Results FeNO,respiratory rate(RR)and serdm IgE level in asthma group were higher than those in control group,and the time to peak expirtory flow/time of expiratory(TPTEF/TE),volume to peak expiratory flow/volume of expiratory(VPEF/VE),tidal volume/kg(VT/KG)and TRACK-C score were lower than those in control group,and the differences were statistically statistically significant(all P<0.05).The FeNO and RR levels in acute stage in the asthma group were higher than those in the remission period,TPTEF/TE,VPEF/VE,VT/KG and TRACKC scores were lower than those in remission stage,and the differences were statistically statistically significant(all P<0.05).Pearson method was used to analyze the correlation between different periods of asthma disease and various observational indicators,and the results showed that different periods of asthma disease were negatively correlated with TPTEF/TE,VPEF/VE and TRACK-C scores(r=-0.468,-0.452,-0.289,all P<0.05).Conclusion TRACK-C score can be fully applied to the condition assessment of 0~3 years old children with asthma,and combined with moisture lung function can better manage asthma in young children.
