摘 要:目的探讨鼻塞式持续气道正压通气(Nasal Continuous Positive Airway Pressure,N-CPAP)对新生儿呼吸衰竭通气功能及预后质量的影响。方法选取2020年7月—2023年7月滕州市妇幼保健院新生儿科收治的90例呼吸衰竭患儿为研究对象,以不同治疗方案为依据分为两组,各45例。对照组患儿接受常规机械通气(Conventional Mechanical Ventilation,CMV)治疗,研究组患儿接受N-CPAP治疗。比较两组患儿治疗效果、相关指标水平及不良事件发生情况。结果研究组治疗总有效率为95.56%,高于对照组的80.00%,差异有统计学意义(χ^(2)=5.075,P<0.05)。治疗后,两组动脉血氧分压、动脉血氧分压/吸入氧浓度、氧合指数及动脉血氧分压/动脉肺泡氧压均更高,二氧化碳分压、呼吸指数及血浆脑钠肽水平均更低,且研究组优于对照组,差异有统计学意义(P均<0.05)。研究组患儿不良事件发生率较对照组更低,差异有统计学意义(P<0.05)。结论NCPAP可有效改善患儿通气功能,在新生儿呼吸衰竭治疗中具有较高的应用价值。Objective To investigate the effects of nasal continuous positive ventilation(N-CPAP)on ventilation function and prognostic quality of neonatal respiratory failure.Methods A total of 90 children with respiratory failure admitted to the Neonatology Department of Tengzhou Maternal and Child Health Hospital from July 2020 to July 2023 were selected as the study objects.They were divided into two groups based on different treatment plans,with 45 cases in each group.The control group was treated with conventional mechanical ventilation(CMV)and the matched group was treated with N-CPAP.The treatment effect,the relevant indexes and the occurrence of adverse events in the two groups were compared.Results The total effective rate of the matched group was 95.56%,which was higher than 80.00% of the control group,the difference was statistically significant(χ^(2)=5.075,P<0.05).After treatment,the levels of arterial partial pressure of oxygen,oxygenation index,arterial partial pressure of oxygen/fraction of inspiration oxygen and arterial oxygen pressure/A-aDO_(2) were higher than those before treatment,while the levels of arterial partial pressure of carbon dioxide,respiratory index and brain natriuretic peptide were lower than those before treatment,and the study group had better levels,the differences were statistically significant(all P<0.05).The incidence of neonatal adverse events in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion N-CPAP can effectively improve the ventilation function of children,and has high application value in the treatment of neonatal respiratory failure.
