摘　　要：目的研究神经特异性烯醇化酶(NSE)、B淋巴细胞刺激因子(BLyS)、增殖诱导配体(APRIL)在病毒性脑炎患者中的表达及与预后的相关性。方法选取2015—2023年河南科技大学第一附属医院收治的118例病毒性脑炎患者,另选取同期体检、经临床生化和脑脊髓穿刺等常规检查确认健康正常的对照组48例。比较不同病情的病毒性脑炎患者血清NSE、BLyS、APRIL水平,不同预后病毒性脑炎患者的相关资料。采用多因素Logistic分析病毒性脑炎预后不良的影响因素,并采取受试者工作特征(ROC)曲线分析血清NSE、BLyS、APRIL单独及联合检测在病毒性脑炎预后评价中的价值。结果重症组血清NSE、BLyS、APRIL水平均高于轻症组及对照组,差异有统计学意义(P<0.05)。预后良好组入院当天及治疗后血清NSE、BLyS、APRIL水平均低于预后不良组,差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,年龄、总住院时间、是否进行机械通气、是否存在癫痫发作以及入院当天血清NSE、BLyS、APRIL水平与病毒性脑炎预后不良有关(OR>1,P<0.05)。ROC曲线显示,入院当天血清NSE、BLyS、APRIL联合检测预测预后不良的AUC值(0.895)高于NSE(0.801)、APRIL(0.781)、BLyS(0.776)。结论血清NSE、BLyS、APRIL在病毒性脑炎患者均呈高表达,三者联合检测可较好地预测患者预后。Objective To investigate the expressions of neurospecific enolase(NSE),B lymphocyte stimulator(BLyS),a proliferation-inducing ligand(APRIL)in patients with viral encephalitis.Methods Totally 118 cases of viral encephalitis admitted to the First Affiliated Hospital of Henan University of Science and Technology from 2015 to 2023 were selected.In addition,48 healthy controls of suspected intracranial infection were selected with physical examination in our hospital and confirmed to be normal by clinical biochemistry and cerebrospinal puncture.Serum NSE,BLyS,and APRIL levels of patients with viral encephalitis with different conditions,and the relevant data of patients with viral encephalitis with different prognosis were compared.Multivariate Logistic was used to analyze the influencing factors for viral encephalitis,and the receiver operating characteristic(ROC)curve was used to analyze the value of serum NSE,BLyS,APRIL alone and combined in the prognosis evaluation of viral encephalitis.Results The serum levels of NSE,BLyS and APRIL in the severe group were higher than those in the mild group and the control group,the difference was statistically significant(P<0.05).The serum NSE,BLyS,and APRIL levels on the day of admission and after treatment were lower than those in the poor prognosis group,the difference was statistically significant(P<0.05).The multivariate Logistic regression analysis showed that age,total length of stay,mechanical ventilation,presence of seizures,and serum NSE,BLyS,and APRIL levels on the day of admission was associated with the poor prognosis of viral encephalitis(OR>1,P<0.05).The results of ROC curve showed that the AUC value of combined of serum NSE,BLyS,and APRIL(0.895)was higher than NSE(0.801),APRIL(0.781),and BLyS(0.776)alone.Conclusion Serum NSE,BLyS and APRIL are highly expressed in patients with viral encephalitis,and their combined detection could predict the prognosis of patients well.
