血清自身免疫性抗体联合检测在1型糖尿病诊断中的价值研究

摘　　要：目的研究血清谷氨酸脱羧酶抗体(Glutamate Decarboxylase Antibodies,GAD-Ab)、胰岛素自身抗体(Insulin Autoantibodies,IAA)、胰岛细胞抗体(Islet cell antibodies,ICA)联合检测在1型糖尿病(Type 1 Diabetes Mellitus,T1DM)诊断中的价值。方法回顾性选取2022年1月—2023年6月烟台市莱阳中心医院诊疗的40例T1DM患者及40名同期来医院体检的健康人群的临床资料,分别设为T1DM组和正常对照组,两组均进行血清GAD-Ab、IAA、ICA检查,研究血清GAD-Ab、IAA、ICA三项联合检测在1型糖尿病诊断中的价值。结果T1DM组血清GAD-Ab、IAA、ICA水平与正常对照组比较明显增高,差异有统计学意义(P均<0.05);三项抗体联合检测诊断T1DM的敏感性(95.00%)、准确性(93.75%)及阴性预测值(94.87)较单项检测明显提高,差异有统计学意义(χ^(2)=24.126,13.628,12.543,P均<0.05)。结论血清GAD-Ab、IAA、ICA三项联合检测明显提高1型糖尿病诊断价值。Objective To study the value of serum glutamate decarboxylase antibodies(GAD-Ab),insulin autoantibodies(IAA)and islet cell antibodies(ICA)in the diagnosis of type 1 diabetes mellitus(T1DM).Methods Clinical data of forty T1DM patients treated in Yantai Laiyang Central Hospital from January 2022 to June 2023 and forty healthy people who came to the hospital for physical examination during the same period were respectively selected,which were respectively the T1DM group and the normal control group.Serum GAD-Ab,IAA and ICA were detected in both groups.The value of serum GAD-Ab,IAA and ICA combined tests in the diagnosis of type 1 diabetes were studied.Results Serum levels of GAD-Ab,IAA and ICA in T1DM group were significantly higher than those in normal control group,and the differences were statistically significant(all P<0.05).The sensitivity(95.00%),accuracy(93.75%)and negative predictive value(94.87%)of combined detection of three antibodies significantly increased of T1DM than single detection,and the differences were statistically significant(χ^(2)=24.126,13.628,12.543,all P<0.05).Conclusion The combined detection of serum GAD-Ab,IAA and ICA can significantly improve the diagnostic value of type 1 diabetes.
