摘 要:目的探讨胰岛素多次皮下注射与持续皮下输注治疗儿童1型糖尿病的效果。方法选取淄博市妇幼保健院于2018年1月—2023年10月收治的90例1型糖尿病患儿作为研究对象,依据电脑随机编号奇偶数分为两组,各45例。对照组采用胰岛素多次皮下注射,观察组为采用持续胰岛素皮下输注,比较两组血糖水平、血糖恢复时间、胰岛素用量、不良事件发生情况。结果治疗3、6个月两组空腹血糖、餐后2 h血糖、糖化血红蛋白均降低,且观察组比对照组更低,差异有统计学意义(P均<0.05)。观察组比对照组的血糖恢复时间更短,胰岛素平均用量、低血糖发生频次更少,差异有统计学意义(P均<0.05)。观察组不良事件发生率为4.44%,低于对照组的28.89%,差异有统计学意义(χ^(2)=9.680,P<0.05)。结论儿童1型糖尿病治疗中持续胰岛素皮下输注可提高血糖控制效果,长久稳定血糖水平,而且血糖恢复快,可减少胰岛素用量,避免相关不良事件的发生,疗效和安全性优于胰岛素多次皮下注射,应用价值显著。Objective To investigate the effect of multiple subcutaneous injection and continuous subcutaneous infusion of insulin in the treatment of type 1 diabetes in children.Methods A total of 90 children with type 1 diabetes received by the Zibo Maternal and Child Health Hospital from January 2018 to October 2023 were selected as the study objects,and they were divided into two groups with 45 cases in each group by computer random number and even number.The control group received multiple subcutaneous injections of insulin,and the observation group received continuous subcutaneous infusion of insulin.Blood glucose level,blood glucose recovery time,insulin dosage and adverse events were compared between the two groups.Results After treatment for 3 and 6 months,the fasting plasma glucose,2 h postprandial blood glucose,glycosylated hemoglobin A1c levels decreased in both groups,the observation group was lower than the control group,and the differences were statistically significant(all P<0.05).The blood glucose recovery time in the observation group was shorter than that in the control group,the average dosage of insulin and the frequency of hypoglycemia were less,and the differences were statistically significant(all P<0.05).The incidence rate of adverse events in the observation group was 4.44%,lower than 28.89%in the control group,and the difference was statistically significant(χ^(2)=9.680,P<0.05).Conclusion In the treatment of type 1 diabetes in children,continuous subcutaneous insulin infusion can improve the blood glucose control effect,stabilize the blood glucose level for a long time,and the blood glucose recovery is rapid,reducing the amount of insulin to avoid the occurrence of related adverse events,and the efficacy and safety are superior to multiple subcutaneous insulin injection,the implement value is significant.
