不同肠内营养剂在老年重症肺炎低白蛋白血症中的应用研究

摘　　要：目的老年重症肺炎出现低白蛋白血症的概率相对较高,这类并发症严重威胁患者生命健康。本文探讨老年重症肺炎低白蛋白血症患者最佳的营养支持方案。方法选取2020年8月—2022年9月四川省内江市第六人民医院收治的100例老年重症肺炎患者,通过抽签方式随机分为对照组与试验组,每组50例。2组患者均采用常规治疗方案和肠内营养支持方案,对照组患者应用肠内营养粉剂,试验组患者则采用肠内营养乳剂,对比2种肠内营养支持方案的应用效果。结果试验组患者的24 h平均血糖和24 h平均血糖峰值均低于对照组,白蛋白、前白蛋白、血红蛋白水平高于对照组,且IL-6、CRP水平均低于对照组,低白蛋白血症发生率也低于对照组,差异均有统计学意义(P<0.05)。结论选用肠内营养乳剂营养支持方案可以显著改善老年重症肺炎低白蛋白血症患者体内应激高血糖情况,有助于降低炎症发生风险,提高营养水平,降低低白蛋白血症的发生率。Objective Hypoalbuminemia is relatively common in elderly patients with severe pneumonia,and this complication poses a serious threat to their health and life.This study aimed to explore the optimal nutritional support plan for elderly patients with severe pneumonia complicated by hypoalbuminemia.Methods A total of 100 elderly patients with severe pneumonia admitted to the Sixth People's Hospital of Neijiang from August 2020 to September 2022 were randomly assigned to a control group and an experimental group,with 50 patients in each group.Both groups received conventional treatment and enteral nutritional support.The control group received enteral nutritional powder,while the experimental group,enteral nutritional emulsion.The application effects of the two enteral nutritional support plans were compared.Results The average 24-hour blood glucose and peak blood glucose levels in the experimental group were lower than those in the control group;the levels of albumin,prealbumin,and hemoglobin were higher in the experimental group than in the control group;the levels of IL-6 and CRP were lower in the experimental group than in the control group,and the incidence of hypoalbuminemia was also lower in the experimental group than in the control group,all with statistically significant differences(P<0.05).Conclusion The use of an enteral nutritional emulsion support plan can significantly ameliorate the hyperglycemia caused by stress in elderly patients with severe pneumonia complicated by hypoalbuminemia,reduce the risk of inflammation,promote nutritional status,and lower the incidence of hypoalbuminemia.
