自拟百丹胃萎汤治疗慢性萎缩性胃炎的临床疗效

摘　　要：目的探讨自拟百丹胃萎汤治疗慢性萎缩性胃炎(CAG)的临床疗效及对血清胃肠激素水平和幽门螺杆菌(Hp)清除率的影响。方法选取2020年6月—2022年6月本院收治的80例CAG患者,随机分为常规组和中药组,每组40例。常规组患者给予奥美拉唑肠溶胶片治疗,中药组患者在常规组基础上给予自拟白丹胃萎汤治疗。比较2组患者治疗前后的中医证候积分,病理性评分,胃肠激素[胃蛋白酶原Ⅰ(PGⅠ)、胃蛋白酶Ⅱ(PGⅡ)、PGⅠ/PGⅡ]水平,治疗后的Hp清除率及临床疗效。结果2组患者治疗后喜温喜按、胃脘痛、乏力气短、大便稀溏、食少纳呆4项中医证候积分均低于治疗前,且中药组患者均低于常规组,差异均有统计学意义(P<0.05)。2组患者治疗后异型增生、肠化生、萎缩、炎症活动、慢性炎症4项病理性评分均低于治疗前,且中药组患者均低于常规组,差异均有统计学意义(P<0.05)。2组患者治疗后血清PGⅠ、PGⅠ/PGⅡ水平均高于治疗前,且中药组患者均高于常规组,差异均有统计学意义(P<0.05)。2组患者治疗前后血清PGⅡ水平比较,差异无统计学意义(P>0.05)。中药组患者治疗总有效率为92.50%,高于常规组的72.50%,差异有统计学意义(P<0.05);中药组患者Hp清除率为95.00%,高于常规组的75.00%,差异有统计学意义(P<0.05)。结论自拟百丹胃萎汤治疗CAG,可有效改善患者临床症状及病理特征,调节胃肠激素水平,提高Hp清除率,效果较好。Objective To explore the clinical efficacy of self-formulated Baidan Weiwei Decoction in the treatment of chronic atrophic gastritis(CAG)and its impact on serum gastrointestinal hormone levels and Helicobacter pylori(Hp)clearance rate.Methods Eighty patients with CAG admitted to our hospital from June 2020 to June 2022 were randomly assigned to a control group and a traditional Chinese medicine(TCM)group,with 40 cases in each group.Patients in the control group were treated with omeprazole enteric-coated tablets,while patients in the TCM group received self-formulated Baidan Weiwei Decoction in addition to the treatment in the control group.The TCM Syndrome Score,pathological scores,levels of gastrointestinal hormones(pepsinogenⅠ[PGⅠ],pepsinogenⅡ[PGⅡ],PGⅠ/PGⅡratio),Hp clearance rate,and clinical efficacy were compared between the two groups before and after treatment.Results After treatment,the scores of four TCM Syndrome items including liking warmth and pressure,epigastric pain,fatigue and shortness of breath,loose stools,and poor appetite and poor digestion were lower in both groups compared to before treatment,with the TCM group showing significantly lower scores than the control group(P<0.05).The scores of four pathological items including metaplasia,intestinalization,atrophy,and chronic inflammation were lower in both groups after treatment compared to before treatment,with the TCM group showing significantly lower scores than the control group(P<0.05).The serum levels of PGⅠand PGⅠ/PGⅡratio were higher in both groups after treatment compared to before treatment,with the TCM group showing significantly higher levels than the control group(P<0.05).No statistically significant difference was observed in the serum PGⅡlevels between the two groups before and after treatment(P>0.05).The overall response rate in the TCM group was significantly higher than that in the control group(92.50%vs.72.50%;P<0.05).The Hp clearance rate in the TCM group was significantly higher than that in the control g
