丹参及其配伍治疗IgA肾病微血管病变的药效作用

摘　　要：IgA肾病病因为虚人外感“热”“瘀”交杂,“热、瘀”损伤脉道迫血妄行,血不归经故临床以血尿多见,“虚、瘀”交错水道不利,故多见水肿。水肿、血尿与现代医学的微血管损伤息息相关,微血管损伤持续存在于IgA肾病病程中。长期的微血管损伤,会导致微循环发生障碍,加重IgA肾病患者的病情;依据中医凉血活血,益气养血的治法,借助凉血活血、益气养血代表药丹参及其配伍对IgA肾病之微血管病变的效应机制进行综述,从而以完善中医血瘀证理论并指导IgA肾病临床治疗。The etiology of IgA nephropathy is a combination of deficiency in the body and external invasion of"heat"and"stasis".The"heat"and"stasis"damage the meridians,causing blood to flow recklessly and not return to the meridians,resulting in the clinical manifestation of hematuria.The intertwining of deficiency and stasis obstructs the water channels,leading to the common occurrence of edema.Edema and hematuria are closely related to microvascular damage in modern medicine,which persists throughout the course of IgA nephropathy.Prolonged microvascular damage can lead to impaired microcirculation and worsen the condition of IgA nephropathy patients.Based on the traditional Chinese medicine method of treatment of blood cooling and blood-activating,replenishing qi and nourishing blood,this article reviews the mechanism of the effects of Radix salviae miltiorrhiza and its compatibility with other herbs on microangiopathy in IgA nephropathy to improve the theory of blood stasis in traditional Chinese medicine and guide the clinical treatment of IgA nephropathy.
