摘　　要：目的:探讨腺苷酸环化酶蛋白-1(CAP1)在慢性阻塞性肺病急性加重期(AECOPD)患者治疗过程中的潜在作用,以评估其作为治疗效果和预后预测指标的可行性。方法:纳入2021年10月—2023年6月我院的66例AECOPD患者,通过ELISA法测量第1、3、5、7天患者血清CAP1的含量;统计患者的预后结果,包括痊愈、好转、恶化和死亡;比较不同时间点的CAP1检测值,使用ROC曲线分析和多因素Logistic回归分析,对CAP1在AECOPD患者中的表达水平与预后结果之间的关联进行深入研究。结果:发现在第1天时,各组之间的CAP1表达差异不显著(F=0.609,P>0.05);在第3、5、7天时,好转、痊愈组的CAP1表达明显高于恶化组和死亡组,差异具有统计学意义(F=49.463、46.711、21.759,P<0.05);ROC曲线分析显示,CAP1在第3天以后的时间点对患者治疗结果的预测能力明显提高(AUC=0.920),具有较高的准确性;相关性分析和多因素Logistic回归分析进一步证实了第3、5和7天的CAP1检测值在预后结果预测中的准确性。结论:血清中CAP1蛋白在AECOPD患者治疗过程中表现出动态变化,其在治疗后期与治疗结果密切相关,CAP1的表达水平可能成为评估治疗效果和预测AECOPD患者预后的指标。Objective:To explore the potential role of CAP1 protein in the treatment of patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD),to evaluate its feasibility as a therapeutic effect and prognostic indicator.Methods:66 AECOPD patients who were admitted to our hospital from October 2021 to June 2023 were included.Measure the serum CAP1 content of patients on 1st,3rd,5th,7th days using ELISA method.Calculate the prognosis of patients,including cure,improvement,deterioration,and death.We compared the CAP1 detection values at different time points and conducted in-depth research on the correlation between the expression level of CAP1 and prognosis in AECOPD patients using ROC curve analysis and multivariate Logistic regression analysis.Results:It was found that at 1st day,there was no difference in CAP1 expression between the groups(F=0.609,P=0.611).On the 3rd,5th,7th days,the expression of CAP1 in the cured group and improvement group were higher than those in the deteriorated group and the death group,with statistical significance(F=49.463,46.711,21.759;P<0.05).Through ROC curve shown that CAP1 significantly improved its predictive ability for patient treatment outcomes at time points after the 3rd day,with AUC 0.920,and high accuracy.Correlation analysis and multivariate Logistic regression analysis further confirmed the accuracy of CAP1 detection values at 3rd,5th,and 7th days in predicting prognosis.Conclusion:The serum CAP1 protein exhibits dynamic changes during the treatment of AECOPD patients,and it is closely related to the treatment outcome in the later stage of treatment.The results of this study indicate that the expression level of CAP1 may become a powerful indicator for evaluating treatment efficacy and predicting the prognosis of AECOPD patients.
