摘  要:目的探讨患者因素对胸部低剂量CT(LDCT)过扫描的影响。方法回顾性收集2022年10月30日至12月3日在杭州市临平区第一人民医院放射科进行LDCT检查的患者467例,评估患者年龄、性别、BMI对肺尖过扫描、肺底过扫描长度的影响。结果存在肺尖过扫描319例(68.3%),优化扫描仅148例(31.7%);存在肺底过扫描239例(51.1%),优化扫描228例(49.9%)。男性患者肺尖过扫描明显多于女性患者(P<0.001)。BMI≥24 kg/m2患者肺尖过扫描明显增多(P<0.01)。其余肺尖和肺底过扫描患者占比比较差异均无统计学意义(均P>0.05)。结论胸部低剂量超解剖范围过扫描是常见的,熟悉患者因素(年龄、性别和体型)及影响程度对个体减少辐射剂量有一定指导意义。Objective To determine patient factors that affect over-scanning in low-dose CT(LDCT)for lung cancer screening.Methods A total of 467 patients who underwent LDCT in the Radiology Department of the First People's Hospital of Linping District from October 30 to December 3,2022,were retrospectively collected to evaluate the effects of age,gender and BMI on the length of over-scanning from above thoracic inlet to lung base.Results There were 319 cases(68.3%)with over-scanning of lung apices,and only 148 cases(31.7%)with optimized scanning;239 cases(51.1%)with lung base over-scanning,and 228 cases(49.9%)with optimized scanning.Among them,male patients had a significantly higher proportion of over-scanning in lung apices than female patients(P<0.001).Number of over-scanning of lung apices in patients with BMI≥24 kg/m2 was significantly increased(P<0.01).Other patients showed no statistical differences in lung apices and base over-scanning(all P>0.05).Conclusion Chest LDCT scanning exceeding the anatomic range is common,and it is of guiding values to understand patient factors(age,sex,and body type)and the degree of influence in reducing exposure dose individually.
