痛风患者血清糖类抗原72-4水平及其与尿酸水平的关系分析

摘　　要：目的分析痛风患者血清糖类抗原72-4(CA72-4)水平,并探讨其与血尿酸水平的关系。方法回顾性选取2020年1月至2022年12月在中国人民解放军联勤保障部队第九〇三医院治疗的痛风患者120例,为痛风组。另择同期来院体检的健康体检者52名为对照组。两组均清晨空腹抽取肘静脉血,采用电化学发光法测定血清CA72-4水平,采用全自动生化分析仪测定血清尿酸水平。比较两组血清CA72-4、尿酸水平;分析痛风组患者血清CA72-4水平与尿酸的相关性及发生痛风的危险因素。结果痛风组患者血清CA72-4、尿酸水平均高于对照组[(5.76±1.39)U/mL比(1.21±0.37)U/mL、(748.29±75.41)μmol/L比(321.20±34.52)μmol/L],差异均有统计学意义(均P<0.05)。Pearson相关分析显示,痛风组患者血清CA72-4水平与尿酸呈正相关(r=0.572,P<0.05)。多因素logistic回归分析显示,高血清CA72-4、尿酸、ESR和hs-CRP水平均是痛风的危险因素(均P<0.05)。结论痛风患者血清CA72-4水平明显升高,且与尿酸水平呈正相关。血清CA72-4水平或可作为痛风患者早期诊断的辅助指标。Objective To analyze the serum carbohydrate antigen 72-4(CA72-4)levels in gout patients and explore its relationship with blood uric acid levels.Methods One hundred and twenty gout patients who were treated at the 903rd Hospital of the Joint Logistics Support Force of the Chinese People's Liberation Army from January 2020 to December 2022 were selected as the gout group.Another 52 healthy individuals who came to the hospital for physical examination during the same period were selected as the control group.The fasting elbow vein blood was collected from the two groups in the morning,serum CA724 levels were measured using electrochemiluminescence,and blood uric acid levels were measured using a fully automated biochemical analyzer.The serum levels of CA72-4 and uric acid were compared between the gout group and the control group;The correlation between serum CA72-4 levels and blood uric acid in patients with gout,and the risk factors for gout were analyzed.Results The serum CA72-4 and uric acid levels of the gout group were both higher than those of the control group[(5.76±1.39)U/mL ratio(1.21±0.37)U/mL,(748.29±75.41)μmol/L ratio(321.20±34.52)μmol/L,and the differences were statistically significant(all P<0.05).Pearson correlation analysis showed a positive correlation between serum CA72-4 levels and uric acid in patients with gout(r=0.572,P<0.05).Multivariate logistic regression analysis showed that high serum levels of CA724,uric acid,ESR,and high sensitivity C-reactive protein(hs-CRP)were all risk factors for gout(all P<0.05).Conclusion The serum CA72-4 level in gout patients is significantly elevated and positively correlated with blood uric acid levels.The serum CA72-4 level may serve as an auxiliary indicator for early diagnosis of gout patients.
