摘 要:目的:分析基于营养风险筛查2002(NRS2002)评分支持结合肠内免疫微生态营养对重症肺炎(SP)患者的肠道微生态及炎症因子的影响。方法:选择本院重症医学科2022年4月—2023年12月收治的98例SP患者,随机数字表法分为两组各49例。对照组给予常规营养支持,观察组基于NRS2002评分支持结合肠内免疫微生态营养。对比两组治疗前后的NRS2002评分、肠道菌群含量、炎症因子、不良风险事件。结果:治疗后两组NRS2002评分、HMGB1、MCP-1、IL-6水平、肠球菌菌落数对数值较治疗前降低,乳杆菌、双歧杆菌、大肠埃希菌菌落数对数值较治疗前升高,且观察组NRS2002营养风险评分、上述炎症因子表达、肠球菌菌落数对数值均低于对照组,乳杆菌、双歧杆菌、大肠埃希菌菌落数对数值均高于对照组(P<0.05)。观察组营养支持相关不良风险事件率低于对照组(P<0.05)。结论:基于NRS2002评分支持结合肠内免疫微生态营养应用于SP患者营养管理中效果显著,可有效改善炎症因子及营养状态,降低不良事件及肠道微生态失衡风险。【Objective】To study the effects of Nutrition Risk Screening 2002(NRS2002)score support combined with enteral immune microecological nutrition the on the gut microbiota and inflammatory factors in patients with severe pneumonia(SP).【Method】Totally 98 patients with SP admitted to the Department of Critical Care Medicine of our hospital from April 2022 to December 2023 were randomly divided into two groups,with 49 cases in each group,using a random number table method.The control group received routine nutritional support,while the observation group received support based on NRS 2002 score combined with intestinal immune microecological nutri⁃tion.Compare the NRS 2002 score,gut microbiota content,inflammatory factors,and adverse risk events before and after treatment between two groups.【Result】After treatment,the NRS2002 scores,HMGB1,MCP-1,IL-6 levels,and logarithmic values of Enterococ⁃cus colonies in both groups decreased compared to before treatment,while the logarithmic values of Lactobacillus,Bifidobacterium,and Escherichia coli colonies increased compared to before treatment.In addition,the NRS 2002 nutritional risk score,expression of inflamma⁃tory factors mentioned above,and logarithmic values of Enterococcus colonies in the observation group were lower than those in the control group,while the logarithmic values of Lactobacillus,Bifidobacterium,and Escherichia coli colonies were higher than those in the control group(P<0.05).The incidence of adverse events related to nutritional support in the observation group was lower than that in the control group(P<0.05).【Conclusion】The application of enteral immune microbiota nutrition based on the NRS 2002 score supportin the nutritional management of SP patients has significant effect,which can effectively improve inflammatory factors and nutritional status,reduce the risk of adverse events and intestinal microbiota imbalance.
