摘　　要：目的:探讨地佐辛复合咪达唑仑辅助局部麻醉在椎间孔镜下髓核摘除术(PELD)中的应用效果.方法:选取2022年1月—2023年6月于玉林市中西医结合骨科医院实施PELD的腰椎间盘突出症(LDH)患者120例作为研究对象,按照随机数字表法分为局部麻醉组、局部麻醉+地佐辛组、局部麻醉+地佐辛复合咪达唑仑组,各40例.比较三组在体位摆好5 min时(T_(0))、局部浸润麻醉时(T_(1))、刺入穿刺针时(T_(2))、置入工作套管时(T_(3))、打磨关节突时(T_(4))、取髓核时(T_(5))的平均动脉压(MAP)、心率(HR)、氧饱和度(SpO_(2))和视觉模拟评分法(VAS)、警觉/镇静(OAA/S)评分.结果:T_(1)~T_(5),局部麻醉+地佐辛复合咪达唑仑组MAP、HR和VAS、OAA/S评分低于局部麻醉+地佐辛组,局部麻醉+地佐辛组MAP、HR和VAS、OAA/S评分低于局部麻醉组,差异有统计学意义(P<0.05).结论:地佐辛复合咪达唑仑辅助局部麻醉在PELD中应用效果较好,可维持LDH患者血流动力学稳定,减轻围术期的焦虑与疼痛.Objective:To investigate the effect of dezocine combined with midazolam assisted local anesthesia in the endoscopic extraction of nucleus pulposus(PELD).Methods:A total of one hundred and twenty patients with lumbar disc herniation(LDH)who received PELD in Yulin Orthopedic Hospital of Integrated Chinese and Western Medicine from January 2022 to June 2023 were selected as the study objects,and were divided into local anesthesia group,local anesthesia+dezocine group,and local anesthesia+dezocine combined midazolam group according to random number table method,with forty cases each.The mean arterial pressure(MAP),heart rate(HR),oxygen saturation(SpO_(2)),visual analogue scale(VAS),and alertness/sedation scale(OAA/S)of three groups at 5 minutes of positioning(T_(0)),local infiltration anesthesia(T_(1)),puncture needle insertion(T_(2)),working cannula insertion(T_(3)),articular process polishing(T_(4)),and nucleus pulposus extraction(T_(5))were compared.Results:The MAP,HR,VAS and OAA/S scores in T_(1)-T_(5),local anesthesia+dezocine combined with midazolam group were lower than those in local anesthesia+dezocine group,and the MAP,HR,VAS and OAA/S scores in local anesthesia+dezocine group were lower than those in local anesthesia group,with statistical significance(P<0.05).Conclusion:Dezocine combined with midazolam assisted local anesthesia has a good effect in PELD,which can maintain hemodynamic stability in LDH patients and reduce perioperative anxiety and pain.
