X线和多排螺旋CT对骨关节创伤的诊断价值探讨

摘　　要：目的:探讨X线和多排螺旋CT对骨关节创伤的诊断价值.方法:选取2021年10月—2023年11月柳州方正司法鉴定所接诊的疑似骨关节创伤患者80例作为研究对象,以手术结果为"金标准",比较两种检查方式的诊断效能.结果:多排螺旋CT诊断符合率高于X线,差异有统计学意义(P=0.049).多排螺旋CT诊断准确度95.0%(76/80)、灵敏度89.5%(68/76)、特异度57.1%(8/14);X线诊断准确度82.5%(66/80)、灵敏度85.5%(65/76)、特异度25.0%(1/4).多排螺旋CT与X线检查在不同骨关节创伤部位的检出率比较,差异无统计学意义(P>0.05).多排螺旋CT图像质量优良率高于X线检查,差异有统计学意义(P=0.029).结论:在骨关节创伤诊断中,多排螺旋CT、X线检查均能准确判断创伤部位,但多排螺旋CT检查诊断效能更佳、图像质量更高,可为治疗方案的制定提供参考依据,具有推广价值.Objective:To explore the diagnostic value of X-ray and multi-slice spiral CT in bone and joint trauma.Methods:A total of eighty patients with suspected bone and joint trauma received by Liuzhou Fangzheng Judicial Appraisal Institute from October 2021 to November 2023 were selected as the research objects,and the diagnostic efficacy of the two examination methods was compared with the surgical results as the"gold standard".Results:The diagnostic coincidence rate of multi-slice spiral CT was higher than that of X-ray,and the difference was statistically significant(P=0.049);the diagnostic accuracy of multi-slice spiral CT was 95.0%(76/80),the sensitivity 89.5%(68/76),and the specificity was 57.1%(8/14).The diagnostic accuracy of X-ray was 82.5%(66/80),the sensitivity 85.5%(65/76),and the specificity was 25.0%(1/4).There was no significant difference in the detection rate between multi-slice spiral CT and X-ray examination in different sites of bone and joint trauma(P>0.05).The image quality of multi-slice spiral CT was higher than that of X-ray examination,and the difference was statistically significant(P=0.029).Conclusion:In the diagnosis of bone and joint trauma,multi-slice spiral CT and X-ray examination can accurately identify the trauma site,but multi-slice spiral CT examination has better diagnostic efficacy and higher image quality,and can provide reference for the formulation of treatment plans,and has the value of promotion.
