摘 要:目的:探讨血清高半胱氨酸蛋白61(CYR61)、干扰素-γ(IFN-γ)、基质金属蛋白酶-9(MMP-9)水平与Graves眼病疾病活动性的关系。方法:选择GO患者112例为研究对象,依据临床活动性评分(CAS)分为活动期组(CAS≥3分,62例)和非活动期组(CAS<3分,50例)。比较两组血清CYR61、IFN-γ、MMP-9水平。分析血清CYR61、IFN-γ、MMP-9与疾病活动性的相关性及影响疾病活动性的危险因素。采用受试者工作特征(ROC)曲线分析血清CYR61、IFN-γ、MMP-9对GO疾病活动性的预测价值。结果:非活动期组患者血清CYR61、IFN-γ、MMP-9水平较活动期组下降(均P<0.05)。血清CYR61、IFN-γ、MMP-9水平与GO患者疾病活动性呈正相关(均P<0.05)。血清CYR61、IFN-γ、MMP-9为影响GO患者疾病活动性的影响因素(均P<0.05)。血清CYR61、IFN-γ、MMP-9对GO患者疾病活动性均有一定的预测价值,三者联合的预测价值更高(均P<0.05)。结论:GO患者活动期血清CYR61、IFN-γ、MMP-9水平明显高于非活动期,与GO患者疾病活动性呈正相关,三者联合检测对GO患者疾病活动性的临床预测价值良好。Objective:To investigate the relationship between serum levels of cysteine-rich protein 61(CYR61),interferon-γ(IFN-γ),matrix metalloproteinase-9(MMP-9)and disease activity in patients with Graves’ophthalmopathy(GO).Methods:According to the clinical activity score(CAS),112 GO patients were divided into the active group(CAS≥3,62 cases)and the inactive group(CAS<3,50 cases).The serum levels of CYR61,IFN-γand MMP-9 were compared between the two groups.The correlation between serum CYR61,IFN-γ,MMP-9 and disease activity,and the risk factors affecting disease activity were analyzed.ROC curve was used to analyze the predictive value of serum CYR61,IFN-γand MMP-9 for GO disease activity.Results:The serum levels of CYR61,IFN-γand MMP-9 in the inactive group were lower than those in the active group(all P<0.05).Serum CYR61,IFN-γand MMP-9 levels were positively correlated with disease activity in GO patients(all P<0.05).Serum CYR61,IFN-γand MMP-9 were the influencing factors of disease activity in GO patients(all P<0.05).Serum CYR61,IFN-γand MMP-9 all had certain predictive value for disease activity in GO patients,and the combined predictive value of the three was higher(all P<0.05).Conclusion:The serum levels of CYR61,IFN-γand MMP-9 in the active stage of GO patients are significantly higher than those in the inactive stage,which are positively correlated with the disease activity of GO patients.The combined detection of the three has a good clinical predictive value for the disease activity of GO patients.
