降脂消胰汤内服与灌肠联合西药治疗高脂血症性胰腺炎疗效及对患者血脂、凝血功能的影响

摘　　要：目的:研究降脂消胰汤内服与灌肠联合西药治疗对高脂血症性胰腺炎(HLP)患者血脂水平、凝血功能及预后的影响。方法:选取128例HLP患者,按随机数字表法分为两组,中医组64例,对照组61例(随访过程脱落3例)。对照组采用常规西药治疗,中医组在此基础上采用降脂消胰汤内服和灌肠治疗,比较两组患者临床疗效、血脂水平、凝血功能和预后情况。结果:中医组HLP患者治疗总有效率为95.31%,高于对照组的83.61%(P<0.05)。治疗后,中医组总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)低于对照组,高密度脂蛋白(HDL)高于对照组(P<0.05)。治疗后,中医组血管性血友病因子(vWF)、凝血酶原时间(PT)、纤维蛋白原(FIB)、D-二聚体(D-dimer)均低于对照组(P<0.05)。中医组禁食禁水时间、住院总时间均短于对照组,SAP转化率低于对照组(P<0.05)。结论:降脂消胰汤内服与灌肠联合西医治疗HLP可提高患者临床疗效,有效改善血脂水平和凝血功能。Objective:To study the effects of oral and enema administration of Jiangzhi Xiaoyi decoction combined with western medicine treatment on blood lipid levels,coagulation function and prognosis in patients with hyperlipidemic pancreatitis(HLP).Methods:A total of 128 patients with HLP were selected,and randomly divided into the TCM group(64 cases)and the control group(61 cases)after excluding 3 cases lost to follow up.Patients in the control group were given conventional western medicine treatment.On this basis,patients in the TCM group were given oral and enema administration of Jiangzhi Xiaoyi decoction.Therapeutic effects,blood lipid levels,coagulation function and prognosis were compared between the two groups.Results:The total effective rate of treatment in the TCM group was higher than that in the control group(P<0.05).After treatment,the levels of total cholesterol(TC),triglycerides(TG)and low-density lipoprotein(LDL)in the TCM group were lower than those in the control group.The level of high-density lipoprotein(HDL)in the TCM group was higher than that in the control group(P<0.05).After treatment,the levels of von Willebrand Factor(vWF),prothrombin time(PT),fibrinogen(FIB)and D-dimer in the TCM group were lower than those in the control group(P<0.05).The duration of fasting for solids and liquids and length of hospital stay in the TCM group were shorter than those in the control group.The SAP conversion rate in the TCM group was lower than that in the control group(P<0.05).Conclusion:Oral and enema administration of Jiangzhi Xiaoyi decoction combined with western medicine treatment can improve the therapeutic effect on HLP,effectively improve blood lipid levels and coagulation function.
