MSCT与MRI诊断胰腺导管内乳头状黏液性肿瘤价值对比分析

摘　　要：目的:对比分析多层螺旋计算机断层摄影术(MSCT)和磁共振成像(MRI)诊断胰腺导管内乳头状黏液性肿瘤(IPMN)的价值。方法:选取2018年6月—2023年7月宁阳县第一人民医院收治的30例IPMN患者为研究对象,以病理结果为金标准,分析MSCT和MRI检查结果的准确率,并对比两种检查方法的检查时间以及良性与恶性患者IPMN分型、部位、长径、壁结节差异。结果:MSCT和MRI检查结果均显示,30例IPMN患者良性19例、恶性11例,与病理诊断结果显示的30例IPMN患者良性19例、恶性11例的结果一致,即MSCT和MRI诊断IPMN良恶性的准确率均为100%,且两种检查方式对病变分型、部位、长径、壁结节的判定一致性较高,差异无统计学意义(P>0.05)。MSCT的检查时间短于MRI,差异有统计学意义(P<0.05)。良性患者IPMN分型及病变部位与恶性患者差异无统计学意义(P>0.05),而良性患者IPMN长径≤30 mm、无壁结节的比例高于恶性患者,差异有统计学意义(P<0.05)。结论:MSCT和MRI诊断IPMN正确率相近,但MSCT扫描时间较短,临床中可根据患者实际病情采取最佳检查方式。Objective To evaluate the diagnostic value of MRI and MSCT in intraductal papillary mucinous neoplasms of pancreas(IPMN).Methods 30 patients with IPMN treated in Ningyang County First People's Hospital from June 2018 to July 2023 were selected as the research objects.The accuracy of MSCT and MRI examination results was analyzed with pathological results as the gold standard,and the examination time of the two examination methods was compared as well as the differences in the types,locations,length diameters and wall nodular of IPMN between benign and malignant patients.Results The results of MSCT and MRI showed that there were 19 benign cases and 11 malignant cases in 30 IPMN patients,which was consistent with the results of pathological diagnosis of 19 benign cases and 11 malignant cases,that is,the correct rate of diagnosis of benign and malignant IPMN by MSCT and MRI was 100%.There was no significant difference between the two methods(P>0.05)in determining the type,location,length diameter and wall nodules of the lesions.The examination time of MSCT was shorter than that of MRI,and the difference was statistically significant(P<0.05).There was no statistical significance in IPMN classification and lesion location between benign patients and malignant patients(P>0.05),while the proportion of IPMN length≤30 mm and no wall nodule in benign patients was higher than that in malignant patients,the difference was statistically significant(P<0.05).Conclusion The accuracy of MSCT and MRI in the diagnosis of IPMN is similar,but the scanning time of MSCT is short,and the best examination method can be adopted according to the actual condition of patients in clinic.
