阴式子宫切除结合盆底重建术治疗Ⅲ~Ⅳ度盆底器官脱垂的效果

摘　　要：目的:分析阴式子宫切除结合盆底重建术治疗Ⅲ~Ⅳ度盆底器官脱垂的效果。方法:选取2018年1月—2021年12月惠水县中医医院诊治的Ⅲ~Ⅳ度盆底器官脱垂患者66例为研究对象,采取随机抽签法分为常规组与研究组,各33例。常规组采用阴式子宫切除联合阴道前后壁修补手术治疗,研究组采用阴式子宫切除联合盆底重建术治疗。比较两组手术相关指标、术后复发率、治疗效果、并发症发生率。结果:两组术中出血量、手术时间、术后残余尿量、尿管留置时间比较,差异无统计学意义(P>0.05);研究组术后复发率低于常规组,差异有统计学意义(P=0.012)。研究组总有效率高于常规组,差异有统计学意义(P=0.016)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:阴式子宫切除结合盆底重建术治疗Ⅲ~Ⅳ度盆底器官脱垂的效果较好,可有效降低术后复发率。Objective:To analyze the effect of vaginal hysterectomy combined with pelvic floor reconstruction in the treatment ofⅢ-Ⅳdegrees pelvic floor organ prolapse.Methods:Sixty-six patients withⅢ-Ⅳdegrees pelvic floor organ prolapse diagnosed and treated in Qiannan Buyi and Miao Autonomous Prefecture Huishui County Hospital of Traditional Chinese Medicine from January 2018 to December 2021 were selected as the study subjects,and were divided into the conventional group and the study group by the randomized lottery method,with thirty-three cases in each group.The conventional group was treated with vaginal hysterectomy combined with anterior and posterior vaginal wall repair surgery,and the study group was treated with vaginal hysterectomy combined with pelvic floor reconstruction surgery.The two groups were compared in terms of surgery-related indexes and postoperative recurrence rate,therapeutic effect,and complication rate.Results:Comparison of intraoperative bleeding,operation time,postoperative residual urine volume,and retention time of urinary catheter between the two groups showed no statistically significant difference(P>0.05);the postoperative recurrence rate of the study group was lower than that of the conventional group,and the difference was statistically significant(P=0.012).The total effective rate of the study group was higher than that of the conventional group,and the difference was statistically significant(P=0.016).Comparison of complication rates between the two groups,the difference was not statistically significant(P>0.05).Conclusion:The effect of vaginal hysterectomy combined with pelvic floor reconstruction in the treatment ofⅢ-Ⅳdegrees pelvic floor organ prolapse is better,which can effectively reduce the postoperative recurrence rate.
