俯卧位通气联合奥司他韦治疗甲型流感的效果

摘　　要：目的:分析俯卧位通气联合奥司他韦治疗甲型流感的效果。方法:选取2021年10月—2023年10月清华大学玉泉医院收治的甲型流感患者66例为研究对象,采取随机分组的方式分为对照组和观察组,各33例。对照组接受奥司他韦抗病毒治疗,观察组接受俯卧位通气联合奥司他韦抗病毒治疗。比较两组炎性反应指标、免疫功能指标、不良反应发生率、治疗效果、恢复情况。结果:治疗后,两组白细胞介素-10、C反应蛋白、白细胞计数水平低于治疗前,观察组低于对照组,差异有统计学意义(P<0.05);治疗后,两组CD3^(+)、CD4^(+)水平低于治疗前,CD8^(+)水平高于治疗前,观察组CD3^(+)、CD4^(+)水平低于对照组,CD8^(+)水平高于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05);观察组治疗总有效率高于对照组,差异有统计学意义(P=0.031);观察组体温复常时间、呼吸道症状消失时间、住院时间短于对照组,差异有统计学意义(P<0.001)。结论:俯卧位通气联合奥司他韦治疗甲型流感的效果较好,可减轻炎性反应,提高免疫功能,促进症状恢复,缩短住院时间。Objective:To analyze the effect of prone ventilation combined with oseltamivir in the treatment of influenza A.Methods:A total of sixty-six influenza A patients admitted to Yuquan Hospital of Tsinghua University from October 2021 to October 2023 were selected as the study objects,and were randomly divided into control group and observation group,with thirty-three cases in each group.The control group received oseltamivir antiviral treatment,and the observation group received prone ventilation combined with oseltamivir antiviral treatment.The inflammatory response indexes,immune function indexes,incidence of adverse reactions,therapeutic effect and recovery of the two groups were compared.Results:After treatment,the count levels of interleukin-10,C-reactive protein and white blood cells in two groups were lower than before treatment,and the observation group was lower than the control group,the difference was statistically significant(P<0.05);after treatment,the levels of CD3^(+)and CD4^(+)in two groups were lower than before treatment,and the level of CD8^(+)was higher than before treatment,the levels of CD3^(+)and CD4^(+)in observation group were lower than control group,and CD8^(+)was higher than control group,with statistical significance(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05);the total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P=0.031);the recovery time of body temperature,disappearance time of respiratory symptoms and hospitalization time in the observation group were shorter than those in the control group,and the difference was statistically significant(P<0.001).Conclusion:Prone ventilation combined with oseltamivir has a good effect in the treatment of influenza A,which can reduce inflammatory response,improve immune function,promote the recovery of symptoms and shorten the length of hospital stay.
