戴明循环模式下的关爱咨询指导对未婚早孕女性无痛人工流产术术后心理状态及避孕措施落实率的影响

摘　　要：目的 探讨戴明循环(PDCA)模式下的关爱咨询指导在未婚早孕无痛人工流产术术后女性中的应用效果。方法 选取我院2020年3月至2021年3月未婚早孕实施无痛人工流产术的患者80例作为研究对象,采用随机数字表法分为对照组、观察组,各40例。对照组采用常规术后护理,观察组在常规术后护理基础上采用PDCA模式下的关爱咨询指导。比较两组干预后心理状态、避孕措施落实情况,干预前后健康生殖知识认知度、服务满意度。结果 观察组焦虑、抑郁、敌对、恐怖、偏执、躯体化、强迫状态、人际关系敏感、精神病性评分低于对照组(P <0.05);干预后随访1年,观察组失访1例、对照组失访2例。观察组避孕措施落实率97.44%(38/39)高于对照组76.32%(29/38),重复流产发生率7.69%(3/39)低于对照组28.95%(11/38)(P <0.05);干预后观察组健康生殖知识认知度高于对照组(P <0.05);观察组服务满意度95.00%(38/40)高于对照组77.50%(31/40)(P <0.05)。结论 PDCA模式下的关爱咨询指导可改善未婚早孕实施无痛人工流产术女性患者的不健康心理状态,提升避孕措施落实率,增强患者认知,同时能提升患者的服务满意度。Objective To explore the effect of care counseling and guidance under the Deming cycle(PDCA) model in women after painless induced abortion.Methods From March 2020 to March 2021,80 unmarried early pregnancy patients who underwent painless abortion in our hospital were selected as the research objects.Randomized numerical table method was used to divide the group into control group,observation group,40 cases each.The control group received routine postoperative care,while the observation group received PDCA model of care counseling and guidance on the basis of routine postoperative care.The psychological status,implementation of contraceptive measures,awareness of healthy reproductive knowledge and service satisfaction were compared between the two groups after intervention.Results The scores of anxiety,depression,hostility,terror,paranoia,somatization,compulsion,interpersonal sensitivity and psychosis in the observation group were lower than those in the control group(P<0.05).One case was lost to follow-up in the observation group and 2 cases in the control group.The implementation rate of contraceptive measures in the observation group was 97.44%(38/39) higher than that in the control group 76.32%(29/38),and the incidence of repeat abortion was 7.69%(3/39) lower than that in the control group 28.95%(11/38)(P<0.05).After intervention,the awareness of healthy reproduction knowledge in the observation group was higher than that in the control group(P<0.05).The service satisfaction of the observation group was 95.00%(38/40) higher than that of the control group 77.50%(31/40)(P<0.05).Conclusions PDCA model of care consultation and guidance can improve the unhealthy mental state of unmarried women who undergo painless induced abortion,improve the implementation rate of contraceptive measures,enhance patients' cognition,and improve patients' service satisfaction.
