甲泼尼龙联合GnRHa治疗子宫内膜异位症合并反复早期自然流产患者的效果

摘　　要：目的:观察甲泼尼龙联合促性腺激素释放激素激动剂(GnRHa)治疗子宫内膜异位症合并反复早期自然流产患者的效果。方法:选取2022年4月至2023年1月该院收治的150例子宫内膜异位症合并反复早期自然流产患者进行前瞻性研究,采用随机数字表法将其分为对照组和研究组各75例。对照组采取GnRHa治疗,研究组在对照组基础上联合甲泼尼龙治疗,比较两组临床疗效,治疗前后腹痛程度[疼痛数字评分法(NRS)]评分、性激素(孕酮、黄体生成素、卵泡刺激素、雌二醇)水平和治疗期间不良反应发生率。结果:研究组治疗总有效率为97.33%(73/75),高于对照组的88.00%(66/75),差异有统计学意义(P<0.05);治疗1、3、6个月后,两组NRS评分均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05);治疗后,研究组孕酮、黄体生成素、卵泡刺激素、雌二醇水平均低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:甲泼尼龙联合GnRHa治疗子宫内膜异位症合并反复早期自然流产患者可提高治疗总有效率,降低NRS评分和性激素水平,效果优于单纯GnRHa治疗。Objective:To observe effects of Methylprednisolone combined with Gonadotropin-releasing hormone agonist(GnRHa)in treatment of patients with endometriosis complicated with recurrent early spontaneous abortion.Methods:A prospective study was conducted on 150 patients with endometriosis complicated with recurrent early spontaneous abortion admitted to the hospital from April 2022 to January 2023.They were divided into control group and study group by using the random number table method,75 cases in each group.The control group was treated with GnRHa,while the study group was treated with Methylprednisolone on the basis of that of the control group.The clinical treatment effect,the abdominal pain degree[numerical rating scale(NRS)]score,the sex hormone(progesterone,luteinizing hormone,follicle stimulating hormone,estradiol)levels before and after the treatment,and the incidence of adverse reactions during treatment were compared between the two groups.Results:The total effective rate of the study group was 97.33%(73/75),which was higher than 88.00%(66/75)of the control group,and the difference was statistically significant(P<0.05).After 1,3 and 6 months of treatment,the NRS scores of the two groups were lower than those before the treatment,those in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of progesterone,luteinizing hormone,follicle stimulating hormone and estradiol in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Methylprednisolone combined with GnRHa in the treatment of endometriosis with recurrent early spontaneous abortion can improve the total effective rate of treatment and reduce the NRS scores and the sex hormone levels.Moreover,it is superior to single GnRHa treatment.
