摘 要:目的 探讨p38丝裂原活化蛋白激酶(p38MAPK)信号通路在右美托咪定(Dex)减轻大鼠肛门切口痛中的作用。方法 24只SPF级雌性SD大鼠按随机数字表法分为空白组、模型组、模型+Dex组、模型+NS组,每组6只。空白组仅给予麻醉;模型组、模型+Dex组、模型+NS组建立肛门切口痛模型,其中模型+Dex组、模型+NS组在吸入麻醉后、建模前30 min分别鞘内注射Dex 1.5μg/kg(50μl)及等量0.75%氯化钠注射液。测定所有大鼠造模前1 h(T_(0))及造模后2 h(T_(1))、4 h(T_(2))、6 h(T_(3))、24 h(T_(4))时术区热辐射躲避潜伏期(TWL)和机械刺激回缩阈值(MWT),痛阈测定结束后采用免疫蛋白印记法检测脊髓p-p38MAPK蛋白表达量。结果 T_(1~4)时,模型组、模型+Dex组、模型+NS组大鼠术区TWL及MWT均较组内T_(0)时降低(P<0.05或P<0.01);与空白组比较,模型组、模型+Dex组、模型+NS组T_(1~4)时术区TWL及MWT均降低(P<0.05或P<0.01);分别与模型组、模型+NS组比较,模型+Dex组T_(1~4)时术区TWL及MWT均升高(P<0.05或P<0.01)。与空白组比较,模型组、模型+Dex组、模型+NS组脊髓p-p38MAPK蛋白表达量均上调(P<0.01);分别与模型组、模型+NS组比较,模型组+Dex组中该蛋白表达量均下调(P<0.05)。结论 预先鞘内注射Dex可通过提高肛门切口痛大鼠术区痛阈,减轻疼痛,其镇痛机制可能与其下调脊髓p-p38MAPK蛋白的表达有关。Objective To investigate the role of p38 mitogen activated protein kinase(p38MAPK)signaling pathway in dexmedetomidine(Dex)alleviating anal incision pain in rats.Methods Twenty four SPF female SD rats were randomly divided into blank group,model group,model+Dex group and model+NS group,with 6 rats in each group.The blank group was only given anesthesia.Anal incision pain models were established in model group,model+Dex group,and model+NS group.The model+Dex group and model+NS group were intrathecally injected with Dex 1.5μg/kg(50μl)and the same amount of 0.75%sodium chloride injection after inhalation anesthesia and 30 minutes before modeling.The thermal radiation escape latency(TWL)and mechanical stimulation retraction threshold(MWT)in the operation area were measured at 1 h before modeling(T_(0))and 2 h(T_(1)),4 h(T_(2)),6 h(T_(3)),24 h(T_(4))after modeling.After the determination of pain threshold,the expression of p-p38MAPK protein in spinal cord was detected by immunoblotting.Results At T_(1~4),TWL and MWT in the operation area of rats in the model group,model+Dex group and model+NS group were lower than those at t 0(P<0.05 or P<0.01).Compared with the blank group,the TWL and MWT of the operation area in the model group,model+Dex group and model+NS group were decreased at T_(1~4)(P<0.05 or P<0.01).Compared with the model group and the model+NS group,the TWL and MWT in the operation area were increased in the model+Dex group at T_(1~4)(P<0.05 or P<0.01).Compared with the blank group,the expression of p-p38MAPK protein in the model group,model+Dex group and model+NS group were up-regulated(P<0.01).Compared with the model group and the model+NS group,the protein expression in the model+Dex group was down regulated(P<0.05).Conclusion Preemptive intrathecal injection of Dex can relieve pain by increasing the pain threshold in the operation area of rats with anal incision pain,and Its analgesic mechanism may be related to the down-regulation of P p38MAPK protein expression in the spinal cord.
