摘  要:目的 比较醋氯芬酸片与布洛芬缓释胶囊对骨性关节炎(OA)患者软骨细胞和基质代谢的影响。方法 选取2021年8月—2022年8月青岛市市立医院与青岛市即墨区人民医院收治的OA患者204例,依据随机数字表法分为布洛芬组与醋氯芬组,每组102例。布洛芬组给予布洛芬缓释胶囊,醋氯芬组给予醋氯芬酸片,2组均连续治疗6周。比较2组治疗前后血清学指标[Dickkopf相关蛋白1(DKK-1)、护骨因子(OPG)、硬骨素(SOST)]、软骨细胞基质代谢指标[软骨细胞中蛋白多糖(PG)、Ⅱ型胶原蛋白]、膝关节功能指标,不良反应。结果 治疗6周后,2组血清DKK-1水平高于治疗前,血清OPG、SOST水平低于治疗前,且醋氯芬组升高/降低幅度大于布洛芬组(P<0.01);2组血清PG水平低于治疗前,血清Ⅱ型胶原蛋白水平高于治疗前,且醋氯芬组降低/升高幅度大于布洛芬组(P<0.01);2组西安大略和麦克马斯特大学骨关节炎指数评分、步行能力低于治疗前,膝关节压痛值、膝关节活动度高于治疗前,且醋氯芬组降低/升高幅度大于布洛芬组(P<0.05或P<0.01)。醋氯芬组不良反应总发生率为5.88%,低于布洛芬组的26.47%(χ^(2)=15.943,P<0.001)。结论 相较于布洛芬缓释胶囊,醋氯芬酸片治疗OA的效果显著,可有效减轻软骨细胞损伤,促进软骨细胞增殖,促使基质代谢趋于正常,提高膝关节功能,缓解疼痛程度,且安全性较高。Objective To compare the effect of aceclofenac tablets and ibuprofen extended-release capsules on chondrocyte and matrix metabolism of OA patients.Methods A total of 204 cases of OA patients admitted to Qingdao Municipal Hospital and the People′s Hospital of Jimo.Qingdao from August 2021 to August 2022 were selected,and they were divided into the ibuprofen group and the aceclofenac group based on the random number table method,102 cases in each group.The ibuprofen group was given ibuprofen sustained-release capsules,and the aceclofenac group was given aceclofenac tablets,and both groups were treated continuously for 6 weeks.Serological indexes(DKK-1,OPG,SOST),chondrocyte matrix metabolism indexes(PG,typeⅡcollagen in chondrocyte),knee function indexes before and after treatment,and incidence of adverse reactions were compared between the two groups.Results After 6 weeks of treatment,serum DKK-1 level of the two groups was higher than that before treatment,serum OPG and SOST levels were lower than those before treatment,and increase/decrease amplitude in the aceclofenac group were greater than those of the ibuprofen group(P<0.01);PG level in chondrocyte of the two groups was lower than that before treatment,typeⅡcollagen level in chondrocyte was higher than that before treatment,and decrease/increase amplitude in the aceclofenac group were greater than those of the ibuprofen group(P<0.01);WOMAC score and walking ability of the two groups were lower than those before treatment,knee joint pressure pain value and knee joint mobility were higher than those before treatment,and increase/decrease amplitude in the aceclofenac group were greater than those of the ibuprofen group(P<0.05 or P<0.01).Total incidence of adverse reactions in the aceclofenac group was 5.88%,which was lower than 26.47%in the ibuprofen group(χ^(2)=15.943,P<0.001).Conclusion Compared with ibuprofen sustained-release capsules,aceclofenac tablets is more effective in the treatment of OA,which can effectively reduce chondrocyte injury,promote th
