摘  要:肠道微生态系统由人体内分布的共生菌群及其生存环境组成,是人体内规模最大、结构最复杂的一种微生态系统。微生物为葡萄糖耐量异常(impaired glucose tolerance,IGT)患者提供了多种生化反应线索,并参与多种生理进程。肠道微生态系统通过改变短链脂肪酸的生成、脂多糖介导的炎症反应和胆汁酸信号的传导等多种途径干预血糖调节机制和胰岛素敏感性。随着肠道微生态治疗策略的不断推出,以运动、微生态制剂、膳食纤维、二肽基肽酶Ⅳ抑制剂和中药为代表的多种疗法在临床中得以应用和推广。本文综述了相关的研究进展成果。可以预见的是,肠道微生态作为新兴的研究领域,有望成为控制IGT发生发展和延缓2型糖尿病进行性加重的又一关键点。Intestinal microecosystem is the largest and most complex microecosystem in the human body, which consists of symbiotic flora and their living environment. Microorganisms provide a variety of biochemical cues for patients with impaired glucose tolerance (IGT) and participate in a variety of physiological processes. The intestinal microecosystem interferes with glucoregulatory mechanisms and insulin sensitivity through a variety of pathways including altered short-chain fatty acid production, lipopolysaccharide-mediated inflammatory responses and bile acid signalling. As therapeutic strategies targeting intestinal microecology continue to be introduced, various therapies represented by exercise, microecological preparations, dietary fibre, dipeptidyl peptidase Ⅳ inhibitors and traditional Chinese medicine have been applied and promoted in clinical practice. This article reviews the relevant research progress and results. Predictably, intestinal microecology as an emerging area of research, is expected to be another key to controlling the onset and development of IGT and delaying the progressive exacerbation of type 2 diabetes.
