摘  要:目的分析糖尿病周围神经病变(DPN)患者的神经电生理指标变化状况及其与病情程度的相关性。方法收集2021年3月至2023年3月宁德市闽东医院收治的97例DPN患者,纳入DPN组,以同期97例健康体检人员作为对照组,所有研究对象进行神经电生理检测,包括腓总神经运动传导速度、波幅,上下肢体感诱发电位(SEP)及皮肤交感反应(SSR)。使用多伦多临床评分系统将97例DPN患者分为重度DPN组和轻中度DPN组,比较两组的神经电生理指标,分析神经电生理指标变化状况与病情程度的相关性。结果DPN组腓总神经运动传导速度、波幅低于对照组,差异有统计学意义(P<0.001)。DPN组上肢SEP异常、下肢SEP异常、SSR异常发生率均高于对照组(P<0.001)。重度DPN患者腓总神经运动传导速度、波幅低于轻中度DPN患者(P<0.001)。重度DPN患者与轻中度DPN患者上肢SEP异常、下肢SEP异常、SSR异常率比较差异有统计学意义(P<0.05)。Spearman相关性分析结果显示,腓总神经运动传导速度、波幅与DPN病情程度呈负相关(r=-0.415、-0.547,P均<0.05)。上肢SEP异常、下肢SEP异常、SSR异常与DPN病情程度呈正相关(r=0.691、717、0.897,P均<0.05)。结论神经电生理指标可提高DPN的诊断率,并能反映患者的病情程度,有助于DPN的早期发现和治疗。Objective To explore the changes of neuroelectrophysiological indexes in patients with diabetic peripheral neuropathy(DPN)and their correlation with disease grade.Methods A total of 97 DPN patients admitted to Mindong Hospital of Ningde City from March 2021 to March 2023 were enrolled into the DPN group,and 97 healthy physical examination subjects were taken as the control group during the same period.Neuroelectrophysiological tests were performed on all subjects,including the common peroneal nerve motor conduction velocity,amplitude,upper and lower limb sensory evoked potential(SEP)and skin sympathetic response(SSR).Using the Toronto clinical scoring system,97 patients with DPN were divided into severe group and mild to moderate group.The neuroelectrophysiological indexes of the two groups were compared,and the correlation between the neuroelectrophysiological indexes and the severity of the disease was analyzed.Results The motor conduction velocity and amplitude of the common peroneal nerve in the DPN group were lower than those in the control group(P<0.001).The incidence of upper limb SEP abnormalities,lower limb SEP abnormalities,and SSR abnormalities in the DPN group was higher than that in the control group(P<0.001).The conduction velocity and amplitude of common peroneal nerve in severe group were lower than those in mild to moderate group(P<0.001).There was significant difference between severe DPN patients and mild to moderate DPN patients in upper limb SEP abnormality,lower limb SEP abnormality and SSR abnormality(P<0.05).Spearman correlation analysis showed that the motor conduction velocity and amplitude of common peroneal nerve were negatively correlated with the severity of DPN(r=-0.415,-0.547,P<0.05).SEP abnormality in upper limb,SEP abnormality in lower limb and SSR abnormality were positively correlated with DPN disease degree(r=0.691,0.717,0.897,P<0.05).Conclusion Neuroelectrophysiological indexes can improve the diagnosis rate of DPN and reflect the severity of the disease,which is helpful for
