摘　　要：目的探讨合并下直肌嵌顿的眶颧复合体骨折的早期手术治疗效果。方法回顾性分析2010年1月至2015年11月我院眼科收治的眶颧复合体骨折合并下直肌嵌顿56例(56只眼)的手术资料和随访结果。结果术后患眼眼球突出度凹陷<2 mm 53例,占94.64%;经过术后功能训练3例患者复视未完全消失,占5.36%,均为周围视野复视;1例患者下睑、鼻翼和上唇的皮肤麻木感未消退,占1.79%;面部外形矫正双侧面部对称,眼眶无塌陷或下移者达52例,占92.86%;双侧面部基本对称,眼眶塌陷或下移2 mm以内4例,占7.14%。张口度在经过术后功能训练后上下切牙切缘间距大于2.5 cm者54例,占96.43%;2例患者残留轻度张口受限,上下切牙切缘间距2~2.5 cm。结论合并下直肌嵌顿的眶颧复合体骨折的早期手术可有效恢复眼球运动受限、复视。改善面部畸形及张口受限等,取得良好效果。Objective To investigate the early surgical correction of orbital zygomatic complex fracture with incarceration and herniation of the rectus inferior and to observe its curative effect.Methods The data of 56 eyes with orbital zygomatic complex fracture with incarceration and herniation of the rectus inferior during January 2010 to November 2015 were reviewed.Results Of 53(94.64%)eyes'enophthalmos.were less than 2 mm;3(5.36%)patients'diplopia was not disappeared.The diplopia was in peripheral visual field.54(96.43%)patients'degree of mouth opening after postoperative function training was more than 2.5cm;52(92.86%)patients'the facial appearance was bilateral symmetry without orbital collapse or downward displacement.Conclusions Early surgical treatment is effective for orbital zygomatic complex fracture with incarceration and herniation of the rectus inferior,it can restore the appearance,eye movements,mouth opening and improve the facial deformity.
