复方鱼腥草合剂治疗急性咽炎疗效观察及对血清炎症因子、免疫功能的影响

摘　　要：目的:观察复方鱼腥草合剂治疗急性咽炎的临床疗效及对血清炎症因子、免疫功能的影响。方法:选取急性咽炎患者150例作为研究对象,随机分为观察组和对照组各75例。观察组给予头孢克肟胶囊联合复方鱼腥草合剂治疗,对照组给予头孢克肟胶囊治疗。2组均连续治疗5 d。比较2组临床疗效、症状消失时间及不良反应,以及治疗前后中医证候评分、细胞免疫功能[外周血T淋巴细胞亚群CD3^(+)、CD4^(+),辅助性T淋巴细胞亚群1 (Th1)、辅助性T淋巴细胞亚群2 (Th2),CD4^(+)/CD8^(+)、Th1/Th2]、血清炎症因子[白细胞介素-6 (IL-6)、干扰素-γ (INF-γ)、C-反应蛋白(CRP)、分泌性免疫球蛋白A (SIgA)、白细胞介素-1β (IL-1β)、肿瘤坏死因子-α (TNF-α)]水平。结果:观察组总有效率为94.67%,对照组为80.00%,2组比较,差异有统计学意义(P<0.05)。观察组咽痛、咽部黏膜红肿、咽部灼热、声嘶、咽喉部异物感消失时间及体温稳定时间均短于对照组,差异均有统计学意义(P<0.05)。治疗后,2组吞咽不利、咽部干燥灼热、咽痛、口渴、恶寒、发热等中医证候评分均较治疗前降低(P<0.05),且观察组上述各项评分均低于对照组(P<0.05)。治疗后,2组外周血CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、Th1、Th1/Th2水平均较治疗前升高(P<0.05),且观察组上述5项指标均高于对照组(P<0.05)。治疗后,2组血清TNF-α、IL-1β、CRP、IL-6水平均较治疗前降低(P<0.05),血清INF-γ、SIgA水平均较治疗前升高(P<0.05);且观察组TNF-α、IL-1β、CRP、IL-6水平低于对照组(P<0.05),INF-γ、SIgA水平高于对照组(P<0.05)。观察组不良反应发生率为10.67%,对照组为6.67%,2组比较,差异无统计学意义(P>0.05)。结论:复方鱼腥草合剂治疗急性咽炎可减轻机体炎症反应,增强免疫功能,显著改善中医证候,加速病情康复,临床疗效显著,且安全性良好。Objective:To observe the clinical effect of Compound Yuxingcao Mixture on acute pharyngitis and its effect on serum inflammatory factors and immune function.Methods:A total of 150 cases of patients with acute pharyngitis were selected as the study subjects and randomly divided into the observation group and the control group,with 75 cases in each group.The observation group received treatment with Cefixime Capsules combined with Compound Yuxingcao Mixture,and the control group was treated with Cefixime Capsules.Both groups were treated continuously for 5 days.The clinical effects,symptom disappearance time,and adverse reactions were comapred between two groups.Traditional Chinese medicine(TCM)syndrome scores,cellular immune function[T lymphocyte subsets in peripheral blood including CD3^(+)and CD4^(+),type 1 helper T cells(Th1 cells),type 2 helper T cells(Th2 cells),CD4^(+)/CD8^(+),and Th1/Th2],and levels of serum inflammatory factors[interleukin-6(IL-6),interferon-γ(INF-γ),C-reactive protein(CRP),secretory immunoglobulin A(SIgA),interleukin-1β(IL-1β),and tumor necrosis factor-α(TNF-α)]were compared before and after treatment.Results:The total effective rate was 94.67%in the observation group,and 80.00%in the control group,the difference being significant(P<0.05).The disappearance time for pharyngeal pain,pharyngeal mucosal redness and swelling,pharyngeal burning,hoarseness,and foreign body sensation in the throat,as well as temperature stabilization time in the observation group were shorter than those in the control group,differences being significant(P<0.05).After treatment,the scores of TCM syndromes such as dysphagia,dry and burning throat,sore throat,thirst,aversion to cold,and fever in the two groups were decreased when compared with those before treatment(P<0.05),and the above scores in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of CD3^(+),CD4^(+),CD4^(+)/CD8^(+),Th1,and Th1/Th2 in peripheral blood in the two groups were increased when compare
