复方苦参熏洗液坐浴对混合痔术后创面愈合、肛门功能恢复的影响

摘　　要：目的:观察复方苦参熏洗液坐浴对混合痔术后创面愈合、肛门功能恢复的影响。方法:选择接受外剥内扎术治疗的混合痔患者85例,按随机数字表法分组为观察组43例和对照组42例。观察组术后给予复方苦参熏洗液坐浴,对照组术后给予0.5%聚维酮碘液坐浴。治疗14 d后,比较2组术后临床疗效,症状缓解时间、肛缘水肿评分及肛门功能恢复情况。结果:观察组总有效率为95.35%,对照组为80.95%,2组比较,差异有统计学意义(P<0.05)。治疗后,观察组创面水肿消退、疼痛缓解、渗出物消失及创面愈合的时间均短于对照组,差异均有统计学意义(P<0.05)。术后2 d,2组肛缘水肿评分比较,差异无统计学意义(P>0.05);术后3、5、7 d,观察组肛缘水肿评分均低于对照组,差异均有统计学意义(P<0.05)。治疗后,2组肛管最大收缩压、肛管舒张压、肛管静息压均较治疗前升高(P<0.05),肛管高压带长度较治疗前缩短(P<0.05);且观察组肛管最大收缩压、肛管舒张压、肛管静息压高于对照组(P<0.05),肛管高压带长度短于对照组(P<0.05)。结论:混合痔术后应用复方苦参熏洗液坐浴治疗可明显缓解术后疼痛,减轻水肿,有助于促进创面愈合及肛门功能恢复。Objective:To observe the effect of hip bath with Compound Kushen Fumigant on wound healing and anal function recovery after operation for mixed hemorrhoids.Methods:A total of 85 cases of patients with mixed hemorrhoids were selected and divided into the observation group and the control group according to the random number table method,with 43 and 42 cases in each group respectively.After operation,the observation group was given hip bath with Compound Kushen Fumigant and the control group was given hip bath with 0.5%Povidone Iodine.After 14 days of treatment,clinical effects,symptom remission time,anal edema scores and anal function recovery were compared between the two groups.Results:The total effective rate was 95.35%in the observation group and 80.95%in the control group,the difference being significant(P<0.05).After treatment,the wound edema remission time,pain relief time,exudate disappearance time and wound healing time in the observation group were shorter than those in the control group,differences being significant(P<0.05).In 2 days after operation,there was no significant difference being found in the comparison of the scores of anal edema between the two groups(P>0.05).In 3,5 and 7 days after operation,the scores of anal edema in the observation group were lower than those in the control group,differences being significant(P<0.05).After treatment,the maximum systolic pressure,diastolic pressure and resting pressure of anal canal in the two groups were increased when compared with those before treatment(P<0.05),and the length of high pressure band of anal canal was shortened when compared with that before treatment(P<0.05);the maximum systolic pressure,diastolic pressure and resting pressure of anal canal in the observation group were higher than those in the control group(P<0.05),and the length of high pressure band of anal canal was shorter than that in the control group(P<0.05).Conclusion:After operation for mixed hemorrhoids,the application of hip bath with Compound Kushen Fumigant can obviously r
