长期服用抗精神病药物治疗的患者心脏超声变化的特点及研究价值

摘　　要：目的:探究长期服用抗精神病药物治疗的患者心脏超声的变化特点及价值。方法:选择本院2017年1月至2021年7月期间收治的80例精神分裂症患者为研究对象,按照患者服药时间将患者分为短期组(≤2个月)和长期组(>1年),分别为33例、47例,比较两组患者左室收缩功能减低程度、心功能不全发生率、心脏超声指标。结果:长期组左室收缩功能减低总发生率较短期组更高,差异有统计学意义(P<0.05);长期组心功能不全发生率(42.55%)高于短期组(9.09%),且差异有统计学意义(χ^(2)=10.598,P<0.05);用药后,两组患者左房内径(LAD)、左室射线分数(LVEF)、E/A均下降,左室舒张末内径(LVEDD)均升高,但短期组优于长期组(P<0.05)。结论:精神分裂症患者长期服用抗精神病药物,其左心结构及心功能发生明显改变,可通过观察患者心脏超声变化,为临床早期干预、防止心功能损害提供科学依据。Objective:To investigate the characteristics and value of echocardiographic changes in patients receiving long-term treatment of antipsychotics.Methods:A total of 80 patients with schizophrenia admitted to the hospital from January 2017 to July 2021 were selected as the research subjects.The patients were divided into short-term group(≤2 months,33 cases)and long-term group(>1 year,47 cases)according to the medication time.The degree of left ventricular systolic dysfunction,the incidence of cardiac insufficiency and echocardiography parameters were compared between the two groups.Results:The total incidence of left ventricular systolic dysfunction in the long-term group was higher than that in the short-term group(P<0.05),and the incidence of cardiac insufficiency(42.55%)was also higher than that in the short-term group(9.09%)(2=10.598,P<0.05).After medication,left atrial diameter(LAD),left ventricular ejection fraction(LVEF)and E/A decreased,and left ventricular end-diastolic diameter(LVEDD)increased.These indicators in the short-term group were better than those in the long-term group(P<0.05).Conclusion:There are significant changes of left ventricular structure and cardiac function in schizophrenia patients who take antipsychotics for a long time.Echocardiographic changes provide a scientific basis for early clinical intervention and prevention of cardiac function damage.
