低分子肝素与利伐沙班对大隐静脉曲张患者术后下肢深静脉血栓的形成及其对炎症因子水平的影响

摘　　要：目的探究利伐沙班与低分子肝素治疗大隐静脉曲张对患者术后下肢深静脉血栓形成与炎症因子水平的影响。方法选取2020年1月—2023年6月通州区二甲中心卫生院外科收治的62例大隐静脉曲张患者作为研究对象。按照随机数表法分为两组,各31例,两组均采用常规治疗,参照组患者术后采用低分子肝素治疗,研究组采用利伐沙班治疗。对比两组患者治疗前后C反应蛋白(C-reactive Protein,CRP)、肿瘤坏死因子-α(Tumor Necrosis Factor-α,TNF-α)、D-二聚体(D-dimer,D-D)、凝血酶时间(Thrombin Time,TT)、纤维蛋白原(Fibrinogen,FIB)等指标变化情况、下肢深静脉血栓发生率、下肢静脉曲张复发率、并发症发生情况。结果治疗后研究组CRP、TNF-α、D-D、TT、血沉、低切血黏度与高切血黏度指标值、并发症发生率均比参照组低,血浆黏度水平、FIB水平比参照组高,差异有统计学意义(P均<0.05)。结论经手术治疗的大隐静脉曲张患者采用低分子肝素和利伐沙班治疗,对其术后下肢深静脉血栓的形成都有预防作用,但利伐沙班治疗大隐静脉曲张可以降低患者的炎症因子水平,改善患者的凝血功能与血流变学指标,且能在一定程度上预防患者术后出现各种并发症,具有较高的治疗效果。Objective To explore the effects of rivaroxaban and low molecular weight heparin on postoperative deep vein thrombosis and inflammatory factors in patients with saphenous varicose vein.Methods A total of 62 patients with varicose veins of great saphenous vein admitted to the surgery department of the second Central Health Center of Tongzhou District from January 2020 to June 2023 were selected as the study objects.According to random number table method,the patients were divided into two groups,31 cases in each group,both groups were treated with conventional treatment,the reference group was treated with low molecular weight heparin after surgery,and the study group was treated with rivaroxaban.Comparison of C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),D-dimer(D-D)in the two groups before and after treatment,thrombin time(TT),fibrinogen(FIB)and other indicators,the incidence of deep vein thrombosis,recurrence rate of varicose veins,and the incidence of complications.Results After treatment,CRP,TNF-α,D-D,TT,ESR,index values of low and high cut blood viscosity and incidence of complications in the study group were lower than those in the reference group,and plasma viscosity and FIB levels were higher than those in the reference group,and the differences were statistically significant(all P<0.05).Conclusion The treatment of low molecular weight heparin and rivaroxaban for varicose veins of the great saphenous vein after surgery has a preventive effect on the formation of deep vein thrombosis of the lower extremities after surgery.However,rivaroxaban for varicose veins of the great saphenous vein can reduce the level of inflammatory factors of the patients,improve the coagulation function and hemorheology indexes of the patients,and can prevent various postoperative complications of the patients to a certain extent,and it has high therapeutic effect.
