依诺肝素钠抗凝联合气压治疗在老年髋部骨折下肢深静脉血栓防治中的疗效分析

摘　　要：目的探讨依诺肝素钠抗凝联合气压治疗在老年髋部骨折下肢深静脉血栓(Deep Venous Thrombosis,DVT)防治中的应用价值。方法单纯随机选取2021年11月—2023年11月都匀市人民医院收治的90例老年髋部骨折手术患者为研究对象,按照随机数表法分为两组,各45例。对照组采用气压治疗,研究组在对照组基础上联用依诺肝素钠抗凝治疗,比较两组疗效。结果研究组下肢DVT发生率为4.44%,低于对照组17.78%,差异有统计学意义(χ^(2)=4.050,P=0.044)。研究组日常活动能力、髋关节功能评分高于对照组,住院时间短于对照组,活化部分凝血酶时间、凝血酶时间、凝血酶原时间长于对照组,D二聚体、纤维蛋白原水平低于对照组,差异有统计学意义(P均<0.05)。研究组患侧和健侧大腿周径差小于对照组,股静脉与腘静脉血流速度快于对照组,差异有统计学意义(P均<0.05)。结论依诺肝素钠抗凝联合气压治疗在老年髋部骨折手术患者下肢深静脉血栓防治中的应用效果显著,能够有效改善凝血功能,保证下肢血液循环正常,促进髋关节功能提升。Objective To study the application value of enoxaparin sodium anticoagulation combined with pneumatic compression therapy in the prevention and treatment of lower extremity deep venous thrombosis(DVT)of hip fracture in the elderly.Methods 90 cases of elderly hip fracture surgery patients admitted to Duyun People's Hospital from November 2021 to November 2023 were simple randomly selected as the study objects and were divided into two groups with 45 cases in each group according to the randomized numerical table method.The control group was treated with pneumatic compression,and the study group was treated with enoxaparin sodium anticoagulation on the basis of the control group,comparing the clinical efficacy of the two groups.Results The incidence rate of lower extremity DVT in the study group was 4.44%,which was lower than 17.78% in the control group,the difference was statistically significant(χ^(2)=4.050,P=0.044).The daily activity ability and hip function scores of the study group were higher than those of the control group,the time of hospitalization was shorter than that of the control group and activated partial thrombon time,thrombin time and prothrombin time were longer than that of the control group,the levels of D dimer and fibrinogen were lower than that of the control group,the differences were statistically significant(all P<0.05).The circumference diameter difference between the affected and healthy side of the study group was smaller than that of the control group,the blood flow rate of femoral vein and popliteal vein was faster than that of the control group,the differences were statistically significant(all P<0.05).Conclusion The application of enoxaparin sodium anticoagulation combined with pneumatic compression therapy in the prevention and treatment of lower extremity deep venous throm bosis in elderly patients undergoing hip fracture surgery has a significant effect,which can effectively improve the coagulation function,ensure the normal blood circulation of the lower extremities,and promot
