铺姜灸治疗抑郁症的疗效观察及对血清5-HT、IL-6、TNF-α水平的影响

摘　　要：目的观察铺姜灸治疗轻中度虚证抑郁症的临床疗效及对血清5-HT、IL-6、TNF-α水平的影响。方法选取2022年3月至2023年5月本院门诊收治的轻中度虚证抑郁症患者80例并随机分组,治疗组40例,采用铺姜灸治疗,对照组40例,采用重复经颅磁刺激治疗(rTMS),两组均治疗6周。对比分析两组治疗前、治疗2周、治疗4周、治疗6周汉密顿抑郁量表(HAMD-17)积分及中医证候症状量表(TSSS)评分及治疗前后患者血清5-HT、IL-6、TNF-α水平,采用HAMD-17积分及TSSS评分减分率进行疗效评定。结果治疗前两组HAMD-17积分和TSSS评分及血清5-HT、IL-6、TNF-α水平检测比较,无统计学差异(P>0.05);治疗2周、4周、6周后HAMD-17积分和TSSS评分组内比较均较治疗前降低(P<0.01);治疗2周、4周、6周后HAMD-17积分和TSSS评分组间比较治疗组均优于对照组(P<0.05);治疗6周后两组较治疗前血清5-HT水平升高(P<0.01),IL-6、TNF-α水平降低(P<0.01);治疗6周后治疗组血清5-HT、IL-6、TNF-α水平变化较对照组变化显著(P<0.05);6周后治疗组总有效率86.49%,对照组总有效率65.71%,治疗组疗效优于对照组(P<0.05)。结论铺姜灸在轻中度虚证抑郁症治疗中疗效良好,铺姜灸可提高抑郁症患者血清5-HT并降低IL-6、TNF-α水平。Objective To observe the clinical efficacy of ginger moxibustion on depression with mild to moderate deficiency syndrome and its influence on serum 5-hydroxytryptamine(5-HT),IL-6 and TNF-αlevels.Methods Eighty patients with mild to moderate deficiency depression who were treated in our hospital were randomly divided into two groups according to the generated random numbers.Patients in the treatment group were treated with ginger moxibustion,patients in the control group were treated with repetitive transcranial magnetic stimulation(rTMS).Both groups were treated for 6 weeks.The scores of Hamilton depression scale 17(HAMD-17)and TCM Syndrome Symptom Scale(TSSS)before treatment,2 weeks,4 weeks and 6 weeks after treatment,and the levels of serum 5-HT,IL-6 and TNF-αbefore and after treatment were compared between the two groups.The HAMD-17 score and TSSS score reduction rate were used to evaluate the efficacy.Results There was no significant difference in HAMD-17 score,TSSS score and serum 5-HT,IL-6 and TNF-αlevels between the two groups before treatment(P>0.05).After 2 weeks,4 weeks and 6 weeks of treatment,the HAMD-17 score and TSSS score were lower than those before treatment(P<0.01).After 2 weeks,4 weeks and 6 weeks of treatment,the HAMD-17 score and TSSS score of the treatment group were better than those of the control group(P<0.05).After 6 weeks of treatment,the levels of serum 5-HT in the two groups were higher than those before treatment(P<0.01),and the levels of IL-6 and TNF-αwere lower than those before treatment(P<0.01).After 6 weeks of treatment,the changes of serum 5-HT,IL-6 and TNF-αlevels in the treatment group were more significant than those in the control group(P<0.05).After 6 weeks,the total effective rate of the treatment group was 86.49%(32/37),and the total effective rate of the control group was 65.71%(23/35).The curative effect of the treatment group was better than that of the control group(P<0.05).Conclusion Ginger moxibustion has a good effect in the treatment of mild to moderate defi
